| 摘 要： | 临床上已经发现强直性脊柱炎（ＡＳ）能导致髋关节的破坏，髋关节可能最后强直，但人们尚不清楚髋关节破坏和强直的原因。本文作者在Ｘ线片测量了８０例和ＡＳ的髋关节，观察了１６例ＡＳ髋关节手术中和病变并取材作了病理检查。从Ｘ线片上两组髋臼和股骨头之比值的对比发现，ＡＳ组的股骨头增大。统计学处理有显著差别（Ｔ＝１９．７７，Ｐ＜０．００１）。病理研究表明，股骨头内纤维组织增生，小血管增生和血管炎非常明显。作者认为，这就是股骨间增大和髋痛的原因。如果髋关节的软骨面长期被增大的股骨头压迫，软骨将发生溶解，髋关节最后强直。

| Abstract: | ItwasobservedthatthehipcanbedestroyedduetoAnkylosingSpondylitis(AS)andthehipmaybestiffinaly.IthasnotknownyetforthereasonoftheAShipdestroyingandstifness.Inthisar-ticle,thefemoralheadandacetabulmweresurveyedwith80casesofnormalandAShipsonX-raypic-tures.16casesofAShipwereoperatedandtheirspecimenswereinspectedpathologicaly.TheX-raysur-veyshowedthattheASfemoralheadsarelargerthanthoseofthecontrolgroup,evaluatingbyratiooftheacetabulmandfemoralhead.(T=19.77,P<0.001).thepathologicalresearchshowedtherearesignificantfibrelastosis,capilarectasiaandcapilaritiswithinthefemoralhead.WeconsiderthatisthereasonwhythefemoralheadbecamelargerandpatientsofAShaveapainhip.Ifchondroofhipispressuredbylargerhead,chondrolysiswiltakeplace.Terefore,thehipbecomesstiffinaly. |
