| 摘 要： | 目的：探索糖尿病链式分层管理模式在社区应用后对患者自我管理水平以及血糖控制情况的影响。方法：2013年10月—2014年9月上海市徐汇区凌云社区卫生服务中心采用2型糖尿病链式分层管理模式对203例2型糖尿病患者进行管理。结果：管理1年后，患者的糖尿病自护行为得分、自我管理水平、空腹血糖、糖化血红蛋白控制水平均有明显提高（P＜0.01）。结论：2型糖尿病链式分层管理模式是有效、积极的管理模式，值得在社区中应用并推广。

Preliminary evaluation of application of the hierarchical chain management mode of type 2 diabetes in a community |

| Abstract: | Objective:To explore the effect of the hierarchical chain management mode of diabetes on self-management level and inlfuence on blood glucose control in patients after it was applied in the community. Methods:From Oct. 2013 to Sept. 2014, 203 cases of type 2 diabetes were managed with the hierarchical chain management mode in Lingyun Community of Xuhui District, Shanghai. Results:After one year management, the score of the self-care behavior, self-management level, fasting blood glucose and glycosylated hemoglobin control levels of the patients were significantly increased(P<0.01). Conclusion: The hierarchical chain management mode is an effective and positive mode, and it is worthy of application in the community. |
