家庭医生签约式服务对患者健康管理的效果分析

目的：研究家庭医生签约式服务在改善患者健康方面的效果，为今后开展社区健康管理服务提供相关依据。方法：2014年9月—2015年9月，随机抽取180名社区患者，对观察组90人提供家庭医生签约式服务，对照组90人提供常规社区健康服务，12个月后对比相关健康指标，并评价家庭医生签约式服务的干预效果。结果：经干预后，观察组与对照组在选择社区门诊就医的比例及认为社区就医费用便宜，差异具有统计学意义（P＜0.05）。观察组与对照组在掌握慢性病常识、愿意改变原有生活行为方式、合理饮食、适量运动、戒烟戒酒、自我监测及情绪调节各方面相比差异均有统计学意义（P＜0.05）。结论：采用家庭医生签约式服务，能增强患者的自我健康保健意识，增加对健康管理的依从性，值得推广应用。

Analysis of the effect of the family doctor contract service on the health management of patients

Objective:To study the effect of the family doctor contract service in improving the health of patients to provide the basis for the development of community health management services in the future. Methods:A total of 180 patients who volunteered to participate in this study were randomly selected from August 2014 to August 2015, the family doctor contract service was provided for the observation group with 90 cases, and the conventional community health service for the control group with 90 cases. After 12 months the related health indicators were compared, and the intervention effect of family doctor contract service was evaluated. Results:Before and after the intervention, the differences in the choice of community outpatient service ratio and the low cost of community medical treatment in the observation group and control one had the statistical signiifcance(P>0.05). The differences in the master of chronic diseases common sense, willing to change the original way of life, reasonable diet, moderate exercise, smoking and alcohol cessation, self monitoring, emotional regulation and all aspects between the observation group and control one had statistical signiifcance(P<0.05). Conclusion:The adoption of family doctor contract service can enhance the patients’ self health care consciousness and increase the compliance of health management, and is worth of popularization and application.
