头痛型癫痫的临床和脑电图分析

摘    要:本文报告85例头痛型癫痫。绝大多数为儿童和青少年。主要的临床症状为反复发作性剧烈头痛,或伴有其他植物神经症状和意识障碍,大部分头颅CT检查未见异常。脑电图检查对诊断和鉴别诊断有重要意义。通过常规检查和诱发试验大多数显示阵发性两侧半球或两额部同步对称性中至极高波幅2~7Hz慢波,阵发性或散发性尖波、尖—慢波、棘波、棘—慢波,少数局限于颞、顶、枕部的棘—慢波发放。本病预后较好。本组72.3%病例经系统服用一种抗癫痫药半年至一年后,头痛基本控制,多次脑电图检查未见典型的痫性放电。两者结合,方可考虑逐渐减药或停药为宜。

The Analysis of Headache Epilepsia Clinical Manifestation and EEG
Abstract:Headache epilepsia is one of the commonest paroxysmal type in the focal autonomic epilepsy seizure. It may easily be confused with the headache due to other causes, so the consideration must be valued carefully. This article reported 85 cases of headache epilepsia. Many of the patients are children and juvenile. Clinical symptoms were usually attack of severe headache repeatedly accompanied with symptoms of other autonomic nerve and the disorder of consciousness. In many of this series, the CT scan of brain showed nothing remarkable. The EEG check has important implication in the diagnosis and differential diagnosis. Through the routing examination and evoke test. most of the cases revealed the paroxysmal bilateral hemispheria or both frontal parts synchrony symmetry midlle- highest amplitude 2-7 Hz slow waves; parosysmal or spordic sharp and sharp-wave complex, spike and spike-wave complex discharging. A number of patients, the spike-slow wave comlex discharging limited to temporal, parietal and occipital parts. These were suggestive of the paroxysmal foci which located at the hypothalamus of the pathologic temporal change and limbic systes. But except for the clinical symptoms of migrain were variable, as a rule, the EEG did not present with typical epileptic electric discharge. The headache epilepsia have better prognosis. In our series, 72. 3% of cases have took an antiepileptic drug for half an year to one year then the headache would have been controlled. The EEG checks did not present with typical epileptic electric discharge. When above have been combined, the drug dose may be reduced, or sloped the medication.
