持续性先兆患者治疗前后发作间期SPECT和脑电图的变化

目的：探讨持续性先兆患者的临床特征、发作间期SPECT、长程EEG及CT／MRI特点及其治疗前后的变化。方法：对35例诊断为持续性先兆患者进行治疗前后发作间期SPECT、同步录像脑电图（V-EEG）检查。结果：本组病例发作间期SPECT治疗前正常灌注8例（23％），异常27例（77％），其中低灌注12例（34％），高灌注15例（43％）。治疗后正常灌注增加6例（1Z％），即正常14例（40％），异常21例（60％），异常者中低灌注8例（23％），高灌注13例f37％），V—EEG治疗前正常1例，异常（棘慢波发放）34例（97％），异常者中局限性异常20例（57％），广泛性异常14例（40％）；治疗后正常增加10例（29％）即正常11例（31％），异常24例（69％），其中局限性异常14例（40％），广泛性异常10例（29％）。结论：持续性先兆以临床和EEG为依据，结合SPECT显像提供的辅助信息，以及CT／MRI等神经影像学结果作出诊断。本组病例经抗癫痴药与中药稳心颗粒联合治疗后，病灶修复率为22％，EEG转化为正常者为31％。

The changes of interictcal SPECT and EEG before and afler treatment in 35 patients with aura continua

Objective：To explore the clinical features of,the patients with aura continua, interictcal SPECT, long-ranged EEG and CT/MRI characteristics. Methods： To examine the change of 35 patients with aura continua during treatment by interictcal SPECT and V EEG change study. Results： In the in terictcal SPECT before treatment 8 cases were normal （23%）, 27 cases abnormal （77%）, including hypoperfusion in 12 cases （34%） and hyperperfusion in 15 cases （43%）. After treatment, 6 patients were found normal （17%）, i. e：normal 14 cases （40%）, abnormal 21 cases （60%）, including hypoperfusion in 8 cases （22. 9%） and hyperperfusion in 13 cases （37%）. Before treatment V-EEG of 34 patients （97%） had spindle slow-wave issue, including local anomalies in 20 cases （57%）, diffuse abnormal in 14 cases （40%）; After treatment, the normal crease （29%）,i. e：normal in 11 patients （31%）,abnormal in 24 cases （69%）, including local abnormal in 14 cases （40%） and diffuse anomalies in 10 （29%）. Conclusions： Patients with aura continua can be treated by clinical diagnosis through EEG as the foundation, and SPECT scan as auxiliary index, combined with CT and MRI imaging results diagnosis. After treatment based on such diagnosis,the repair rate is 22 % and EEG of 31% patients EEG can become normal .
