2μm激光“刀削面”式汽化切除术治疗BPH

目的：探讨2μm激光“刀削面”式切除术治疗BPH的疗效。方法：对210例BPH患者由同一术者应用70WRevoLix 激光系统进行手术治疗。结果：手术时间35～140min，平均（62±19）min。术中输血6例，平均输血250ml。术后留置导尿管1～6天，平均（2．5±1．8）天。住院时间为4～7天，平均（4．5±1．2）天。3例于术后出现尿道狭窄，行间断扩张尿道后，排尿症状改善。术后随访3～6个月，术前与术后6个月平均最大尿流率分别为（6．8±2．5）ml／s、（18．5±3．6）ml／s；剩余尿分别为（149．4±70．2）ml／s、（23±6．8）ml／s；IPSS分别为（22．4±5．5）、（5．3±3．2）；QOL分别为（4．8±0．8）、（1．6±1．1），四项指标手术前后比较差异均有统计学意义（P〈0．01）。结论：2p．m激光“刀削面”式汽化切除术简便易行，切除效率高，是一种安全、有效的手术方法。

Technical Properties of the Sliced Noodles-Vaporesection using the Revolix 2 Micron Continuous Wave Laser for the Treatment of Benign Prostatic Hyperplasia

Objective：To analyze the technical properties of transurethral＇ sliced noodles-vaporesection＇ of prostate in the management of benign prostatic hyperplasia. Methods：210 patients with obstructive BPH were treated transurethrally by the＇ sliced noodles-vaporesection＇ technique using the Revolix 70 W 2 micron continuous laser with only one operator. Results： Mean operation time was （62±19） （35-140） min. 6 patients need blood transfusion during the operation, mean blood transfusion volume was 250ml. Mean catheter time was （2.5 ±1.8） （1 6） d. Mean hospitalization time was （4.5±1.2） （4-7）d. all patients were followed-up for 3 to 6 months. Mean Qmax increased from （6.8 ± 2.5） ml/s preoperatively to （18. 5± 3.6 ） ml/s 6 months postoperatively. IPSS and QOL Score improved from （22.4±5.5） and （4.8±0.8）to （5.3±3.2） and （1.6±1.1） six months postoperatively respectively（P〈0.05）. There were 3 cases had external urethral strictures after operation,and treated with urethral dilatation successfully. Conclusions：The method of ‘liced noodles-vaporesection＇of the prostate using the 2 micron laser system is a rapid and effective procedure in the treatment of BPH with high feasibility and safety.
