液基细胞学与组织病理学诊断子宫颈病变的对照分析研究

目的评价子宫颈液基细胞学(TCT)与组织病理学诊断结果的符合率及其在子宫颈病变诊断中的价值。方法采用ThinPrep液基细胞学检查,对2009年6月至2011年6月间,子宫颈细胞学诊断为意义不明的非典型鳞状上皮细胞(ASCUS)以上阳性病例与其组织病理学诊断结果进行比较分析。结果共完成细胞学检查总例数14 111例,阳性869例,阳性率6.16%。其中细胞学检查并且有组织学诊断结果的ASCUS以上异常者440例,细胞学结果为低度鳞状上皮内病变(LSIL)/高度鳞状上皮内病变(HSIL)/ASCUS/不能除外高度病变(ASC-H),阳性检出率为31.9%、96.3%8、.0%、41.7%;LSIL/HSIL其与组织病理学结果符合率分别为68.1%8、5.2%。结论 TCT技术应用于宫颈病变筛查,能有效提高宫颈癌前病变及宫颈癌的检出率,其诊断结果与病理组织学诊断的符合率较高,是子宫颈病变筛查的有效方法。

A comparative study on thin-preparation cytological test (TCT) and histopathological examination in diagnosis of cervical lesions

Objective To evaluate the coincidence rate between cytological examination and result of histopathological examination and the significance of thin-preparation cytological test（TCT） in cervical screening.Methods The abnormal cytological examinations of four hundred and forty cases were compared with those of histopathological results from the Department of Pathology in this hospital between June 2009 and June 2011.Results The results of TCT diagnosis in these patients were LSIL／HSIL／ASCUS ／ASC-H,their positive detection rates were 31.9%,96.3%,8.0% and 41.7% respectively;and the coincidence rates of LSIL／HSIL compared with pathological examination were 68.1% and 85.2% respectively.Conclusions The screening with TCT increases the detection rate of precancerous cervical lesions and cervical cancer.It plays an important role in effectively screening the cervical lesions.
