2型糖尿病患者脉搏波传导速度及其相关危险因素分析

摘    要:目的探讨踝.肱脉搏波传导速度(BaPWV)与2型糖尿病患者相关代谢因素之间的关系。方法选取458例2型糖尿病患者,询问相关病史,测定BaPWV及总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL—C)、高密度脂蛋白胆固醇(HDL—C)、尿酸(UA)、空腹血糖(FPG)、糖化血红蛋白(HbAlc)、超敏C反应蛋白(hs—CRP)、空腹血清胰岛素(Fins)、24小时尿白蛋白、颈动脉内膜中层厚度(IMT)等指标,分析BaPWV与上述各指标的相关性。结果(1)BaPWV与2型糖尿病患者年龄、糖尿病病程、腰围、收缩压、空腹胰岛素、HOMA—IR、hs—CRP、24小时尿白蛋白及IMT呈正相关(P〈0.05),女性患者BaPWV显著高于男性,高血压患者BaPWV显著高于血压正常者(P均〈0.05);(2)进一步按病程分组,发现病程〉10年的患者,其BaPWV显著高于初发患者、病程〈5年及5一10年者(P〈0.05)。结论BaPWV与2型糖尿病患者年龄、病程、血压等具有良好的正相关性。因此,早期发现糖尿病、严格控制血糖、血压对于预防及延缓糖尿病大血管并发症的发生发展有重要意义。

Study of brachial-ankle pulse wave velocity and related metabolic factors in type 2 diabetes patients

Abstract:Objective To investigate the relationship between brachial-ankle pulse wave velocity (BaPWV) and other metabolic factors in type 2 diabetes. Methods A total of 458 subjects with type 2 diabetes were enrolled in the study. Serum fasting plasma glucose(FPG) ,total cholesterol(TC) ,triglycer- ide (TG) ,low density lipoprotein-cholesterol ( LDL-C), high density lipoprotein-cholesterol ( HDL-C ), uric acid ( UA), hemoglobin A1 c ( HbA 1 c), high sensitivity C-reactive protein ( hs-CRP), and serum fasting in: sulin(Fins) ,24 h urinary albumin, carotid intima-media thickness (IMT)and brachial-ankle pulse wave velocity (BaPWV) were collected. Results ( 1 ) BaPWV were significantly correlated with age, duration of T2DM ,Waist circumference ~SBP,FIns,HOMA-IR, hs-CRP,24 h urinary albumin and IMT ( P 〈 0.05 ) ; Compared with the male group, BaPWV were significantly higher in the female group, as well as higher in hypertension group than non-hypertension group ( P 〈 0.05 ). ( 2 ) BaPWV increased with duration of T2DM. In subjects with a duration 〉 10 years,BaPWV were significantly higher than those new-diagnosed and duration ~〈 5 years,5 ~ 10 years (P 〈 O. 05). Conclusions BaPWV were significantly correlated with age ,duration and blood pressure in T2DM patients. Therefore, early glycemic and blood pressure control is important for the prevention of arterial stiffness and vascular complications in T2DM.
