摘    要:目的原发全面性癫痫(idiopathic generalized epilepsy,IGE)患者的脑部常规磁共振成像常难以发现异常显示。文中采用磁共振弥散张量成像(diffusion tensor imaging,DTI)技术观察IGE患者的脑微观的异常结构,探讨其对IGE的神经病理生理学发生机制的意义。方法30例IGE患者为研究对象,30例健康成人为对照组。用1.5Tesla MR扫描仪行DTI数据采集。分别计算平均弥散系数(mean diffusivity,MD)图及部分各向异性(fractional anisotropy,FA)图,采用SPM2软件对2组受试者的MD和FA图进行随机效应分析的两样本t检验,观察患者与正常人相比校正后的MD值及FA值有显著性改变(P<0.01)的脑区。结果与正常人相比,IGE患者双侧丘脑、颞叶、岛叶、枕叶及中脑等脑区MD值降低。内侧前额叶MD值升高。双侧丘脑及双侧扣带回后部FA值升高。双侧内侧前额叶、双侧颞叶等区域FA值降低。结论DTI是一种敏感的影像学技术,可发现IGE患者脑内广泛的微观结构异常区域,可用于癫痫脑网络结构的研究,有助于对原发全面性癫痫病理生理学发生机制的了...
