残总比对慢性阻塞性肺疾病诊断的临床意义

目的:探讨残气容积/肺总量比值(RV/TLC)对慢性阻塞性肺疾病(COPD)辅助诊断的临床意义,寻求以RV/TLC用于诊断COPD的最适参考值。方法:回顾性分析我院2010年5—12月行肺功能检查的209例肺功能检查患者,其中:COPD患者组130例,非COPD患者组79例。记录性别、年龄等一般情况以及RV、TLC、1秒率等指标,以1秒率作为"金标准"计算RV/TLC诊断COPD的敏感度、特异度、阳性似然比、阴性似然比、Youden指数,并绘制ROC曲线。结果:ROC曲线下面积为0.792,其最佳诊断临界点52.8%,敏感度为73.1%,特异度为73.4%。结论:RV/TLC在COPD诊断、鉴别诊断、初筛中有临床意义。

The Clinical Significance of Inspiratory Capacity in Chronic Obstructive Pulmonary Disease

Objective: To explore the clinical significance and to find out the best cut-off point of residual volume/total lung capacity (RV/TLC) in diagnosis of chronic obstructive pulmonary disease (COPD). Methods: From May to December 2011, 209 patients who underwent conventional pulmonary function tests were divided in two groups (with 130 COPD patients in the COPD group and 79 non-COPD patients in the non-COPD group), general characteristics (age and sex) were compared between groups and the receiver operating characteristic curve (ROC) was used to test the sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, and Youden index, FEV1/FVC was used as the "gold standard". Result: The area under the receiver operating characteristic (ROC) curve is 0.792, in the best cut-off point of 52.8%, the sensitivity is 73.1% and the specificity is 73.4%. Conclusion: RV/TLC is of potential clinical significance of COPD, it can be used for diagnosis, differential diagnosis, primary screening of COPD.
