高频电波刀治疗宫颈病变的临床应用

摘    要:目的:评价高频电波刀行电圈切除术(LEEP)治疗宫颈病变中的临床应用价值。方法;回顾性分析我院253例LEEP治疗宫颈病变的临床资料。所有病例均在宫颈病变外缘5mm进出电极,均行宫颈涂片、阴道镜下多点活检和LEEP术后病理诊断,同时观察手术时间、手术出血量及术后疗效。结果:LEEP手术治疗宫颈疾病一次成功率为98.02%(248/253),平均手术时间6.2min,出血量5.6ml,无继发性出血和感染的发生,手术标本经病理学检查明确诊断。结论:LEEP治疗各类宫颈疾病操作简单、安全、省时、出血少,术后阴道排液少,治疗效果佳,且可提供完整的病理标本,对宫颈微小浸润癌和原位癌的早期诊断价值高。

Abstract:objective To evaluate loop electrosurgical excision procedure(LEEP) in the treatment of cervical diseases. Method Two hundred and fifty-three patients with cervical diseases who underwent LEEP in clinic from January 2003 to June 2004 were analyzed retrospectively.All the paients were treated by excision of 5mm outside from the outskirts of the cervical lesions following cytology,colposcopy and biopsy,meanwhile we calculated the cure rate,the operative time,bleeding volume. Results The cure rate of LEEP was 98.02%,the mean operative time was 6.2 minutes,the mean bleeding volume was 5.6 ml.No secondary bleeding and post-operative infection occurred.All these surgical specimens were checked successfully for pathology diagnoses. Conclusion The advantages to use the LEEP to management of the cervical diseases include its simpleness,safety,short operative time,less bleeding, less vaginal discharge,and high cure rate.LEEP can offer intact sample for pathological diagnosis,and has special value for the early diagnosis of the micro-invasive carcinoma of uterine cervix.
