超声造影定量分析在肝脏肿瘤诊断中的应用

摘    要:目的探讨超声造影定量分析在肝脏肿瘤诊断中的应用价值。方法超声造影检查92例患者,共102个肝脏病灶,其中59个恶性,43个良性;记录病灶增强方式、开始强化时间、消退时间、时间-强度曲线(TIC)达峰时间;应用Logistic多因素回归分析上述指标与肝脏局灶性病变的关系,对开始强化时间、消退时间、TIC达峰时间用ROC曲线分析。结果肝脏恶性病灶与良性病灶在动脉相强化模式、门脉相回声及整体强化的差异有统计学意义(P〈0.05)。动脉相20s强化为病灶良恶性诊断标准,敏感性为85.0%,假阳性为14.1%;以时间强度曲线达峰时间33s为诊断标准,敏感性81.4%,假阳性为10.2%。结论超声造影动脉相早期强化作为肝恶性肿瘤诊断指标有临床价值,结合其他指标有助于提高对肝脏恶性肿瘤的诊断。

Abstract:Objective To explore the application value of contrast - enhanced ultrasound in diagnosis of liver tumors by quantitative analysis. Methods Ninety - two patients with 102 focal liver lesions (59 malignant, 43 benign) were examined by contrast - enhanced ultrasound. Then the enhancement patterns, enhancement time, regression time and peak time of time - intensity curve (TIC) were recorded. The relationship between the results and liver lesion was analyzed by logistic multiple factor regression analysis, then the enhancement time, reghession time and peak time of TIC were analyzed by ROC curve. Results There were significant differences in arterial phase enhancement patterns, portal phase echo and entire enhancement between the benign lesions and the malignant lesions group ( P 〈 0.05 ). With early arterial phase enhancement less than 20 s as the diagnostic standard, the sensitivity and false positive rate was 85.0% and 14.1% respectively, while the peak time of TIC was 33 s as the diagnostic standard, the sensitivity and false positive rate was 81.4% and 10.2% ,respectively. Conclusion The enhancement in early arterial phase of liver tumor has important value in diagnosis of malignant tumor in clinic, combined with other factors, it can improve the diagnosis of liver malignant lesions.
