60例术后早期炎性肠梗阻的临床观察

目的:探讨腹部术后早期炎性肠梗阻(EPISBO)的临床特点及诊治方法。方法:将我科收治的60例术后早期炎性肠梗阻的患者随机分为观察组和对照组,对照组给予胃肠减压、抗炎、维持水电解质平衡等方法,观察组在对照组基础上给予中药汤剂复方大承气汤灌肠。观察两组患者症状改善、恢复排气、排便时间、平均住院时间。结果:观察组保守治愈29例,中转手术1例,治愈率为96.7%,平均住院时间为(8.5±3.1)d。对照组保守治愈25例,中转手术5例,治愈率为83.3%,平均住院时间为(13.5±5.6)d。两组患者治愈率及住院时间比较,差异有显著性意义(P<0.05);排气、排便时间比较差异有意义(P<0.05)。结论:术后早期炎性肠梗阻好发于污染性腹腔手术,只要能够做到及时准确的诊治,保守治疗效果令人满意。

The Clinical Observation of 60 Cases of EPISBO

Objective:To observe the clinical features,diagnosis and treatment of the abdomen EPISBO.Methods:Admitted to our department 60 cases of EPISBO were randomly divided into two groups,the control group and observer group,the control group were given gastrointestinal decompression,anti-inflammatory,maintain water and electrolyte balance,the observer group on the basis of the control group given herbal medicine decoction enema compound Dachengqi soup.Symptoms were observed to improve,restore exhaust,defecation time,the average length of stay.Results:Conservative observer group were cured in 29 cases,surgical intervention in 1 case,the cure rate was 96.7%,the average hospital stay was(8.5±3.1)days.Control group,25 patients treated conservatively,surgical intervention in 5 cases,the cure rate was 83.3%,average length of stay(13.5±5.6)days.Two groups of patients cure rate and duration of hospitalization,tbe difference was significant(P<0.05);exhaust,defecation significant difference(P<0.05).Conclusions:EPISBO occurs in contaminated abdominal surgery,as long as able to do timely and accurate diagnosis,conservative treatment was satisfactory.
