不同阶段糖尿病肾病与血浆同型半胱氨酸相关关系的研究

目的;探讨血浆同型半胱氨酸(Hcy)与2型糖尿病(DM)病人合并早期糖尿病肾病的关系。方法:应用化学免疫分析法测定27例正常人及65例不同阶段糖尿病肾病患者血浆同型半胱氨酸水平。结果:①糖尿病正常白蛋白尿组血浆同型半胱氨酸的水平显著高于正常组(P<0.01);②微量白蛋白尿组血浆同型半胱氨酸水平明显高于正常白蛋白组(P<0.01)而显著低平临床白蛋白尿组(P<0.01)③合并高血压者血浆Hcy水平明显高于未合并者(P<0.01)。结论:在微量白蛋白尿阶段前已存在着内皮损伤并伴随肾脏损伤的加重而血浆Hcy水平升高,尤其在高血压组更明显。

The Relationship between Plasma Homocysteine and Different Stages of Diabetic Nephropathy

Objective:To investigate the relationship between plasma homocysteine(Hcy) and early diabetic nephropathy in Type 2 diabetes.Methods:Plasma homocysteine levels were measured by Chemiluminescence Immunoassay Analysis in 27 healthy persons and 65 patients in different stages of diabetic nephropathy.Results:①Plasma homocysteine levels in normal albuminuria with diabetes group were significantly higher than in healthy group(P<0.01);②Plasma homocysteine levels in microalbuminuria group were significantly higher than normal albuminuria group(P<0.01),but significantly lower than clinical macroalbuminuria group(P<0.01);③Plasma homocysteine levels in patients combined with high blood pressure were significantly higher than those with normal blood pressure(P<0.01).Conclusions:Patients in microalbuminuria stage have already exist endothelium injury,and plasma homocysteine levels increase following the aggravation of kidney injury,especially in patients combined with high blood pressure.
