悬雍垂腭咽微波成形术治疗阻塞性睡眠呼吸暂停低通气综合征

摘    要:目的:探讨腭咽微波成形术治疗阻塞性睡眠呼吸暂停低通气综合征((OSAHS)的临床疗效。方法:37例0S—AHS患者均行悬雍垂腭咽微波成形术,其中3例单行腭咽微波成形术,29例加双侧腭扁桃体切除术,5例加双侧腭扁桃体切除术及舌根微波消融术。结果:37例患者术后未出现腭咽关闭不全。19例术后有咽部异物感,持续3~4个月渐消失。6个月后随访,治愈7例(18.92%),显效19例(51.35%),有效7例(18.92%),无效4例(10.81%),总有效率89.19%。术后6个月测量软腭长度较术前明显缩短(〈0.01)。结论:悬雍垂腭咽微波成形术手术时间短、出血少,病人痛苦小,术后效果满意,不惜为治疗0SAHS一种有效的方法。

Abstract:Objective:To investigate the effect of uvulopalatopharyngoplasty(UPPP) by microwave treatment for obstructlve sleep apnea-hypopnea syndrome(OSAHS).Methods:Thirty-seven cases of OSAHS were performed UPPP by using microwave.Among them 3 cases received microwave palatopharyngoplastyonly.Twenty-nine cases underwent bilateral palatine tonsillectomy and another 5 cases underwent bilateral palatine tonsillectomy and tongue base microwave ablation apart from microwave pharyngoplasty.Results:No case appeared velopalatal insufficiency.Nineteen patients felt foreign body in pharyngeal portion,which disappeared after 3~4 months.According to the evaluation standards set in Hangzhou Conference,follow-up was carried out after 6 months.It was found that 7 cases(18.92%)were cured;19(51.35%)showed a significant effect;7(18.92%) appeared effect and 4(10.81%)appeared no effect;and the total effect rate was 89.19%.Conclusion:Uvulopalatopharyngoplasty by using microwave needs 1ess time,shows less hemorrhage and pain and good effects.It is a effective method.
