通腑活血汤治疗脓毒症胃肠功能障碍临床研究

目的观察通腑活血汤治疗脓毒症胃肠功能障碍患者的临床疗效,并探讨其干预机制。方法将89例脓毒症胃肠功能障碍患者随机分为2组。对照组43例予西医常规治疗,治疗组46例在对照组治疗基础上加用通腑活血汤治疗。20 d后观察患者血浆肿瘤坏死因子-α（TNF-α）、脑钠肽（BNP）、血浆降钙素原（PCT）、C反应蛋白（CRP）及白细胞介素-10（IL-10）的水平变化,并进行疗效比较。结果治疗组总有效率97.8%,对照组95.3%,2组总有效率比较差异有统计学意义（P〈0.05）,治疗组疗效优于对照组;2组治疗后血浆TNF-α、BNP、PCT、CRP、IL-10水平与本组治疗前比较差异有统计学意义（P〈0.05）,且2组治疗后组间比较差异均有统计学意义（P〈0.05）。结论通腑活血汤可以明显降低患者血浆TNF-α、BNP、PCT、CRP及IL-10水平,保护胃肠黏膜屏障,促进血液循环,改善患者胃肠功能障碍。

Observation of Tongfu Huoxue decoction on sepsis with gastrointestinal dysfunction

Objective To investigate the effect of Tongfu Huoxue decoction on sepsis with gastrointestinal dysfunction and to explore its mechanism of intervention. Methods 89 cases of sepsis with gastrointestinal dysfunc- tion were randomly divided into control group （n = 43 ）, which was treated by conventional Western medicine and treatment group, which was treated by Tongfu Huoxue decoction on the basis of conventional Western medicine. The changes of levels of TNFct, BNP, PCT, CRP and IL - 10 in blood plasma were observed after 20days. Results The total effective rate in treatment group （97.8%） was higher than that in control group （95.3% ,P 〈0.05）. The differences of TNFot,BNP,PCT,CRP and IL - 10 in blood plasma between before and after treatment group were sig- nificant in two groups （P 〈 0.05 ）. The differences of TNFot, BNP, PCT, CRP and IL - 10 in blood plasma between two groups were significant after treatment （P 〈 O. 05）. Conclusion Tongfu Huoxue decoction can reduce the lev- els of TNFot,BNP,PCT, CRP and IL- 10 in blood plasma, protect the gastrointestinal mucosal barrier, and promote blood circulation and improve the patients with gastrointestinal dysfunction.
