通心络胶囊对2型糖尿病合并冠心病患者血管内皮功能的影响

摘    要:目的观察通心络胶囊对2型糖尿病合并冠心病患者血糖、血脂及血清内皮素-1(ET-1)、一氧化氮(NO)、一氧化氮合酶(NOS)水平的影响,探讨其对血管内皮功能的影响。方法将120例2型糖尿病合并冠心病患者随机分为2组,对照组60例予西医常规治疗,治疗组60例在对照组治疗基础上加通心络胶囊,另选60例健康人作为正常组。治疗2个月。检测3组治疗前后血糖、血脂、血压及血清ET-1、NO、NOS水平变化情况,测定血管内皮功能指标内皮依赖性舒张功能(EDD)及内皮非依赖性舒张功能(EID)。结果治疗组和对照组治疗前空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbAlc)、甘油三酯(TG)、收缩压、舒张压及ET-1水平与正常组比较均明显升高(P<0.05),NO、NOS水平及EDD、EID明显降低(P<0.05)。治疗组和对照组治疗后FPG、2 h PG、HbAlc、TG及ET-1水平与本组治疗前比较均明显降低(P<0.05),NO、NOS水平及EDD、EID显著升高(P<0.05)。治疗组治疗后收缩压及舒张压与本组治疗前明显降低(P<0.05),且收缩压、舒张压、ET-1、NO、NOS、EDD及EID改善均优于对照组(P<0.05)。结论 2型糖尿病合并冠心病患者存在明显的血管内皮功能紊乱,通心络胶囊可通过影响ET-1、NO及NOS的释放改善血管内皮功能。

Abstract:Objective To explore the effects of Tongxinluo on serum glucose, lipid, ET - 1, NO and NOS in patients with DM2 complicated with coronary heart disease. Methods The 120 type 2 diabetes melfitus complicated with coronary heart disease were randomly divided into treatment group and control group : the 60 of them were treated with routine western medicine ( control group) and the other 60 patients ( treatment group) were treated with Tongxinluo on the basis of routine medicine for two months. Another 60 cases of healthy people were as a normal group. Serum ET-1 ,NO,NOS were measured at the following time before and after the treatment by ELISA, nitrate reductase method, enzymatic colorimetric method, respectively. Results FPG, 2 h PG, HbAlc, TG, systolic blood pressure, diastolic blood pressure and ET - 1 levels before treatment of treatment group and control group were signifi- cantly increased compared with those in normal group (P 〈 0.05 ), NO, NOS levels, EDD and E1D were significant lower than those in the normal group (P 〈 0.05 ). FPG, 2 h PG, HbAlc, TG and ET - 1 levels after treatment of treatment group and control group were significantly decreased compared with those before treatment of treatment group and control group ( P 〈 0.05 ) , NO, NOS levels, EDD and EID were significantly increased ( P 〈 0.05 ). Sys- tolic and diastolic blood pressure after treatment were significantly decreased in comparison with those before treatment in treatment group ( P 〈 0.05 ), improvement of systolic and diastolic blood pressure, ET - 1, NO, NOS, EDD and EID in treatment group was superior to that in control group after treatment ( P 〈 0.05 ). Conclusion Type 2 diabetes mellitus complicated with coronary heart disease have serious endothelium dysfunction. Tongxinluo can improve vascular endothelial function by regulating release of ET - 1, NO and NOS.
