糖化血清白蛋白在2型糖尿病人群中检测的临床价值

摘    要:目的探讨在不同血糖控制情况的2型糖尿病患者糖化血清白蛋白(GA)与糖化血红蛋白(HbAlc)的相关性。方法将299例2型糖尿病患者按血糖控制情况分为3组,1组血糖控制相对平稳108例,2组血糖控制稳定情况不详99例,3组住院前2周内有应激情况,血糖短期变化92例,测定各组GA、HbAlc、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、各时点指尖血糖、动态血糖监测(CGMS)及尿糖定量等,并运用现代统计技术作相关分析。结果 3组GA与HbAlc之间有良好的相关性(P<0.001),且r值有递减的趋势;1、2组GA与FPG、2 h PG、CGMS均值、尿糖定量及3组GA与FPG、2 h PG、尿糖定量均有良好相关性(P<0.001,P<0.01,P<0.05),且以1组相关性最好(P<0.001)。1组GA、HbAlc与各时间点血糖及CGMS均值均有相关性(P<0.001)。血糖控制越平稳,GA与HbAlc的相关性越好。结论 GA是一种可反映血糖近期变化的良好指标,与HbAlc及各项血糖指标均有良好的相关性和在一定范围内呈现良好的线性关系,且在血糖相对平稳情况下线性关系最好。

Abstract:Objective To investigate correlation between serum glycated albumin (GA) and HbAlc, under different blood glucose control. Methods 299 2 - DM patients were enrolled and divided into 3 groups according to blood glucose control in the past two weeks before being in hospital. There were 108 cases with a relatively stable blood glucose control in group 1. There were 99 cases with unknown stable blood glucose control in group 2. There were 92 cases with blood glucose short - term changes in stressful situations in 2 weeks prior to admission group 3. GA, HbAlc, FPG, 2 h PG, fingertip blood glucose, CGMS and urine quantitative were measured and analyzed. Re- suits There was a good correlation between GA and HbA1 c ( P 〈 0. 001 ) in group 3, and there is a diminishing trend in r values. There were good correlations between GA, FPG, 2 h PG, CGMS and urine quantitative in group 1 and 2, there were good correlation between GA, FPG, 2 h PG and urine quantitative in group 3 ( P 〈 0. 001, P 〈 0.01, P〈0.05). There was correlation between GA, HbAlc and day fingertip blood glucose spectrum, CGMS in group 1 (P 〈 0.001 ). The more stable blood glucose control, the better the correlation between GA and HbA1 c. Conclusion GA is an excellent index which could reflect the recent status of diabetic blood glucose change. It is signifi- cantly highly correlated with HbAlc and other index and has linear correlation with them in a definite range, further- more, better linear correlation in group of relatively stable.
