MUC2、CDX2在大肠肿瘤中的表达及临床意义

目的 探讨黏蛋白MUC2、CDX2在正常大肠黏膜、大肠腺瘤及大肠腺癌组织中的表达及其与临床各个病理因素之间的关系,并且研究MUC2与CDX2表达之间的相关性.方法 应用免疫组织化学方法对30例大肠腺瘤、60例大肠腺癌及相应正常大肠黏膜30例进行黏蛋白MUC2、CDX2检测,并进行Kaplan-meier生存分析.采用Spearman等级相关分析MUC2与CDX2之间的相关性.结果 在大肠正常黏膜、腺瘤、腺癌中MUC2阳性率分别为100%、93.3%、58.3%;CDX2为90.0%、76.7%、31.7%.在大肠腺癌中,MUC2的表达与肿瘤的浸润深度、淋巴结转移、Dukes分期有关(P<0.05);CDX2的表达与分化程度有关(P<0.01).Kaplan-meier生存曲线及Log Rank显著性检验发现,MUC2阳性表达组s年生存率明显增高(P<0.01).通过Spearman等级相关分析表明,MUC2与CDX2呈正相关(r=0.285,P=0.028).结论 在大肠腺癌中,MUC2与CDX2具有明显相关性,MUC2、CDX2的下调表达与大肠肿瘤的发生有关,可作为鉴别大肠良恶性肿瘤的指标.MUC2与大肠癌的生物学行为有关,并且可作为判断预后的生物学标志物之一.
