人脑星形细胞瘤多体素^1H磁共振波谱研究

摘    要:目的检测并分析不同级别的星形细胞瘤的MRS结果,评价MRS在星形细胞瘤临床的应用价值。方法对星形细胞瘤患者共21例进行磁共振成像和磁共振波谱检查,分析MRS特点,对MRS测定的代谢物浓度进行统计学分析。结果星形细胞瘤MRS较其正常侧表现为NAA峰降低,Cho峰升高,Cr峰较稳定,MI峰散见升高。Lac峰在高级别星形细胞瘤中明显升高。随星形细胞瘤恶性程度上升,NAA/Cr、NAA/Cho减小,Lac/Cr、Cho/Cr升高。NAA/Cho在低级别和高级别星形细胞瘤间差异有统计学意义。Lac/Cr在WH03级与4级星形细胞瘤间差异有统计学意义。结论多体素MRS有助于星形细胞瘤术前分级,NAA/Cho、Lac/Cr最具鉴别意义。

Abstract:Objective Evaluate the application value of multiple MRS in human astrocytoma. Methods Perform the MRS examination in 21 patients who had astrocytoma. Analysis the MRS character. Statistic the metabolic products. Results Lower NAA and higher Cho was seen in the MRS of astrocytoma. Lac significantly increased in high grade astrocytoma. Cr was stable in different side and grade. MI sometimes increased in the astrocytoma. With the step up of pathologic grade, Cho/Cr and Lae/Cr increased, while NAA/Cr and Naa/Cho decreased. There is a significant difference of rqAA/Cho between the low grade and the high grade astrocytoma. A significant difference of Lac/Cr exists in WHO grade 3 and grade 4 astrocytoma. Conclusion MRS is a useful method to diffrentiat human astrocytoma grade with calculating the NAA/Cho ratios and the Lae/Cr ratios.
