摘    要:目的:探讨Ⅰ期切除吻合治疗左半结肠癌性梗阻的方法及围手术期处理原则。方法:回顾性分析1997年2月~2007年10月Ⅰ期切除吻合治疗左半结肠癌急性梗阻42例临床资料。结果:术后42例均痊愈出院,无吻合口瘘,6例并发切口感染,1例并发胃排空障碍。结论:左半结肠癌性梗阻,术中肠减压和结肠灌洗,Ⅰ期切除吻合是安全可靠的,围手术期营养支持、高效广谱抗生素应用、手术技术提高等是预防吻合口瘘的关键。

Abstract:Objective:The purpose of this study was to evaluate the procedure's choice and perioperative management of the left colon cancer with acute obstruction.Methods:The clinical materials of 42 cases of the left colon cancer with acute obstruction operated upon one-stage resection and primary anastomosis between Feb 1997 and Octber2007 were retrospectively reviewed.Results:All of the 42 cases were cured,There were no leaks.Of the 42cases 6 case occurred incision infection,1 case occurred dyfunction of the stomach peristalsis.Conclusions:One-stage resection and primary anastomosis of the left colon cancer with acute obstruction is on the safe side,but the indication must be commanded strictly.We believed that it is the key to enhance the on-tabs proximal decompression and lavage of the colon,perioperative nutrition management,available broad-spectrum antibiotic intravenation and perfect technique of operation fordecreasing.anastomosis fistula of the one-stage resection and primary anastomosis of the left colon cancer with acute obstruction.
