老年类风湿关节炎68例临床特点分析

目的研究老年类风湿关节炎的临床特点。方法对68例年龄≥60岁的老年类风湿关节炎（ERA）及69例非老年类风湿关节炎（NERA）患者的临床症状和实验室检查等项目进行对比分析。结果与非老年类风湿关节炎相比,老年类风湿关节炎组男性发病率比女性高,但差异无统计学意义。老年类风湿关节炎组抗CCP抗体阳性率及抗角蛋白抗体（AKA）阳性率增高,且两组比较差异有统计学意义（P〈0.05）,类风湿因子（RF）阳性率两组比较差异无统计学意义。CRP增高比率ERA组增高,且两组比较差异有统计学意义（P〈0.05）。ESR增快及血小板增高比率,ERA组较NERA组低,差异有统计学意义。结论抗CCP抗体、抗角蛋白抗体（AKA）在诊断老年类风湿关节的特异性优于RF,在一定程度上抗CCP抗体、AKA有助于老年类风湿关节炎的早期诊断,CRP对老年类风湿关节炎的疾病活动具有更大的临床意义。

Analysis on clinical characteristics of 68 elderly patients with rheumatoid arthritis

Objective To study the clinical characteritis of elder patients with rheumatoid arthritis（RA）.Methods Sixty-eight cases in age range≥60-year-old suffered from RA（ERA） and sixty-nine cases of non-elderly RA（NERA） were studied,their clinical symptoms and results of laboratory examinations commonly used for such disease were analyzed.Results In comparison with NERA group,ERA had a higher proportion of male prevalence,but their difference was no statistically significant.Their positive rate of anti-CCP antibody and AKA was higher than that of patients with NERA,their difference was statistically significant.（P0.05）.Their positive rate of RF was no higher than that of patients with NERA,their difference was no statistically significant.（P0.05）.Their CRP was higher than that of patients with NERA,their difference was statistically significant.（P0.05）.Conclusion Anti-ccp antibodys and AKA had higher specificity than RF.In a certain extent,anti-ccp antibodys and AKA may contribute to early diagnosis of ERA.CRP had more clinica significan in disease activity of ERA.
