腹腔镜与传统开腹肝切除术的护理比较

[目的 ]比较腹腔镜肝切除术与开腹肝切除术护理的异同。[方法 ]前瞻性对比分析我院2 0 0 3年 10月— 2 0 0 4年 9月行腹腔镜肝切除术及同期行开腹肝切除的病例各 2 0例 ,比较两组病人手术时间、术中出血量、术后恢复情况及疼痛程度。 [结果 ]腹腔镜组病人的手术时间略长 ,术中出血量较开腹组多 ,但差异无统计学意义 (P >0 .0 5 ) ;腹腔镜组病人下地活动时间及肛门排气时间早于开腹组 ,住院时间明显缩短(P <0 .0 1) ;腹腔镜组病人术后疼痛程度明显低于开腹组 (P <0 .0 5 ) ;两组病人引流量及体温变化差异无统计学意义 (P >0 .0 5 )。 [结论 ]了解两种手术方式临床护理的异同点 ,对提高腹腔镜肝切除围手术期病人的护理质量 ,促进病人康复具有重要的意义。

Nursing comparison on patients underwent hepatectomy through laparoscope and traditional per abdomen

Objective: to compare on both similarities and differences of nursing care of patients undergoing hepatectomy through laparoscope and per abdomen. Method: a total of 40 cases admitted from October 2003 to September 2004 were selected and investigated prospectively. Among them, 20 cases were underwent hepatectomy through laparoscope, the other 20 cases were accepted hepatectomy per abdomen. Indexes including the duration of operation, bleeding quantity during operation, recovery after operation and degree of pain of all cases were compared with each other. Result: the duration of operation was a bit longer, and the bleeding quantity was a little more in patients of laparoscope group than that in traditional way per abdomen group, but there was no statistic difference between them (P>0.05). Both the off-bed ambulation time and the anal aerofluxus time were earlier in laparoscope group than that in per abdomen group. And the hospitalization time of patients in laparoscope group was shorter than that in traditional per abdomen group (P<0.01). The pain degree of patients in laparoscope group was less than that in per abdomen group (P<0.05). There were no differences in terms of drainage quantity and body temperature of the two groups patients (P>0.05). Conclusion: it has important significance to know both similarities and differences of clinical nursing care for patients undergoing the two ways of hepatectomy, in order to enhance the nursing quality for peri-operative patients with hepatectomy via laparoscope and promote their post operative rehabilitation.
