经阴道子宫切除术与经腹子宫切除术的临床效果比较

目的 对比分析经阴道子宫切除术与经腹子宫切除术的临床效果。方法 将2011年1月—2013年12月间采用经阴道（TVH）与经腹（TAH）两种不同方式进行手术子宫切除的患者85例,分为TVH组患者45例、TAH组患者40例;观察记录两组患者手术时间、术中出血量、术后排气时间、术后镇痛率、术后并发症及住院时间等,对比分析两种术式临床效果。结果 两组患者均顺利完成手术,术中无盆腔脏器损伤,术中出血量无显著差异（P〉0.05）;TVH组手术时间、术后平均住院时间、首次排气时间、下床时间、Hb差值均明显小于TAH组,差异有统计学意义（P〈0.05）;术后经阴道组各类术后并发症发生率均低于经腹组,但组间差异不显著（P〉0.05）。结论 经阴道子宫切除术在手术时间、创伤以及术后康复方面比经腹手术更具优势,在把握好临床手术指征前提下可作为子宫切除的首选术式。

Clinical effect comparison of vaginal hysterectomy and abdominal hysterectomy

Objective To investigate clinical effect of vaginal hysterectomy and abdominal hysterectomy.Methods Totally 85 patients in our hospital from January 2011 to December 2013 were divided into total vaginal hysterectomy（TVH）group（45） and total abdominal hysterectomy（TAH） group（40）.The Operative time,blood loss,postoperative discharge time,the rate of postoperative analgesia,postoperative complications and length of hospital stay of two groups were observed and analyzed.Results After treatment,patients of two groups were successfully operated,surgery without pelvic organ injury and no significant difference in blood loss（P〉0.05）.The operative time,postoperative length of stay,the first exhaust time,bed time,Hb difference of TVH group was significantly lower than that of the TAH group（P〈0.05）.Postoperative complications of TVH group were lower than that of the TAH group,but no significant difference between groups（P〉0.05）.Conclusion Vaginal hysterectomy at the time of surgery,trauma and postoperative rehabilitation has advantages over abdominal surgery,in a good grasp of the premise of the clinical indications for surgery as the preferred surgical hysterectomy.
