剖宫产术后腹壁切口子宫内膜异位症临床治疗分析

目的 探讨剖宫产术后腹壁切口子宫内膜异位症患者的早期诊断及临床治疗方法。方法 收集驻马店市第一人民医院2008年11月至2014年2月收治的剖宫产术后腹壁切口子宫内膜异位症92例患者,根据临床治疗方式的不同随机分为观察组（50例）和对照组（42例）,观察组患者采用手术联合药物治疗,对照组采用药物保守治疗,对比两组患者的临床疗效。结果 观察组患者总有效率为100.00%,随访2年2例患者复发,复发率为4.00%;对照组患者总有效率为76.19%,2年后20例患者复发,复发率为47.62%。两组患者治愈率与复发率比较有显著性差异（P〈0.05）。治疗前两组患者经期切口疼痛情况及切口肿块体积变化情况基本一致,对比无统计学意义（P〉0.05）;治疗后观察组患者症状及体征基本得到改善,明显优于对照组患者的改善情况,差异有统计学意义（P〈0.05）。结论 剖宫产术后腹壁切口子宫内膜异位症根据典型临床症状、腹部手术史及B超检查可早期做出诊断,手术是治疗本病的首选方法,联合药物治疗可降低临床复发率。

Clinical analysis of cesarean abdominal incision endometriosis

Objective To investigate the early diagnosis and clinical treatment of abdominal incision cesarean endometriosis patients.Methods Our hospital from November 2008 to February 2014 were treated abdominal incision cesarean endometriosis 92 patients,depending on the clinical treatment of randomized into observation group（50 cases） and control group（42 cases）,the observation group were treated with surgery combined with drug therapy,the control group received conservative treatment,clinical efficacy compared two groups of patients.Results The total effective rate was100.00% of patients,followed for 2 years and 2 patients recurrence rate was 4.00%;total effective rate of the control group were 76.19%,after 2 years and 20 patients with recurrence rate was 47.62%.There was a significant difference（P〈0.05）.Two groups of patients before treatment of menstrual pain conditions incision and wound tumor volume changes are basically the same,there was no significant（P〉0.05）;observation group after treatment,the patient＇s symptoms and signs of fundamental improvement,the improvement was significantly better than the control group.The difference was statistically significant（P〈0.05）.Conclusion The abdominal incision after cesarean section endometriosis according to typical clinical symptoms,abdominal surgery and B-ultrasound can make early diagnosis,surgery is the preferred method of treatment of this disease.
