全肺灌洗治疗矽肺与其它尘肺

本文报告全肺灌洗治疗矽肺与其它尘肺的技术改进、临床效果以及灌洗回收液(BALF)各项分析与细胞培养观察的结果。我们于1986.9～1989.3为44例各期矽肺与10例尘肺施行104次大容量单侧全肺灌洗,未发生任何合并症。灌洗后胸闷与气急的改善率为92.5%与81.6%,肺功能轻度好转者占33.3%。BALF 内含大量(10~7～10~9)已吞噬矽尘的肺泡巨噬细胞,超微结构与生化学检查以及细胞培养证实该巨噬细胞能分泌致纤维化介质,促进成纤维细胞增殖。作者认为:全肺灌洗用于矽(尘)肺防治是安全、有效的疗法。

Whole lung Lavage as an Effective Treatment of Silicosis and Other Pneumoconioses

A modified technique of whole-lung lavage,its clinical effect and resultsobtained by various analyses of lung lavage effluent and cell culture were re-ported.104 massive unilateral whole-lung lavages in 44 silicotic patients and10 patients with other pneumoconioses from Sep 1986 to Mar 1989 were per-formed.No complications occured.Postlavage,92.5% and 81.6% of the na-tients had their breathing discomfort and dyspnea ameliorated.Lung functiontest showed slight improvement in 33.3%.Number of silica ladenmacrophages found in the BALF was 10~7-10~9.Ultrastructural andbiochemical examimations as well as observations on cell culture demonstratedthat the macrophages were able to secret specifically synthesized mediators,which were highly potenty in stim(?)lating fibroblast proliferation.It can beconcluded that whole-lung lavage is a safe and effective treatment for silicosisand other pneumoconioses.
