超选择多支血管药物灌注治疗股骨头缺血性坏死

目的:探讨股骨头多支供血动脉内药物灌注治疗股骨头无菌性坏死的手术方法及初步疗效。方法:行股动脉穿刺插管,将4F导管送至旋股内,外侧动脉,闭孔动脉分支(圆韧带动脉),进行DSA造影,注入尿激酶50万单位、脉络宁注射液40 ml及654-2注射液30 ml等。术后口服抗凝药物,静滴脉络宁及肝素,肌注谷泰康灵,以加强疗效。结果:通过对60个髋关节治疗后临床观察,患者疼痛消失和明显减轻91.7%,减轻者8.3%,关节功能恢复到正常或一级者为88.3%,治疗后:股骨头区供血血管明显增粗、增多、延长。结论:本疗法能明显改善股骨头血液供应,促进股骨头坏死的修复和重建。是一种简便、安全可靠的疗法。

Treatment of avascular necrosis of femoral head with super-selective multi-arteries infusion of medicine

Objective: To discuss the surgery method and preliminary curative effect about medicine perfusion therapy for avascular necrosis of the femoral head.Methods: A line of femoral head puncture inserts a tube,delivers the 4F drive pipe to via circumfiex and external femoral arteries and obturator artery(round ligament artery),carries on the DSA radiography,and followed by slow injection of medicines via the catheter including the urokinase 500,000 units,vein rather inoculation fluid 40 ml and 654-2 inoculation fluid 30 ml and so on.After operation,intravenous administration of orally prevents clotting the medicine,the static drop vein rather and the heparin,the valley tranquillity spirit to strengthen the curative effect.Results: Through the observation of 60 cases clinical treatment,the patient ache vanishes with obviously reduces 91.7%,the alleviator 8.3%,the joint function restores to is normal or a level is 86.7%,after treatment: The blood of femoral head donates obviously to increase.Conclusion: This treatment can improve the femoral head blood supply obviously,and can promote the necrosis repair and reconstruction of the femoral head.It is a very effective and safe treatment.
