胃肉瘤样癌的组织学及免疫组化观察

目的探讨胃肉瘤样癌的病理形态学及免疫组化表型。方法对2例胃肉瘤样癌进行形态学和免疫组化观察，并结合文献探讨该肿瘤的病理学表现。结果2例肿瘤均为溃疡型，镜下图像以梭形细胞组成的肉瘤样结构为主，表面为小细胞癌和管状腺癌成份，两种成份有移行，梭形细胞成份Vimentin强阳性、NSE强阳性，局部CK18阳性、S-100阳性、CgA阳性、Syn阳性。结论胃肉瘤样癌罕见，该肿瘤侵袭性强、预后差。

Clinical obsenation of histological and of sarcomatoid carcinoma of stomach

Objective To investigate the clinicopatholngic features and immunohistochemical phenotypes of sarcomatoid carcinoma of the stomach. Methods Two case of the sarcomatoid carcinoma were observed by morpholngic and imrnunohistochemical approaches , with its phenotypes compare with those of the reported cases . Results The tumor was found at the stomach , mainly composed of spindle cells. Tubular adenocarcinoma and small cell carcinoma were observed in the superficial portion , with transition to the spindle cells in the deeper part. The spindle cell components were strongly positive for vimentin and NSE , with focal expression of CK18,S- 100,CgA and Syn . The lesion was diagnosed as sarcomatoid carcinoma . Conclusions Sarcomatoid carcinoma of the stomach is rare . The neoplasm is highly aggressive and its prognosis is poor .
