细胞DNA倍体分析结合高危型HPV、支原体和衣原体检查与宫颈上皮内瘤变相关性研究

摘    要:目的探讨宫颈细胞DNA倍体、高危型HPV、支原体和衣原体检测结果与宫颈上皮内瘤变的相关性。方法将265例育龄妇女宫颈细胞DNA倍体检查阳性者分为感染因素阳性(高危型HPV阳性、UU/Mh阳性、CT阳性)和感染因素阴性(高危型HPV阴性、UU/Mh阴性、CT阴性),再将感染因素阳性者分为单因素感染(包括高危型HPV感染或UU/Mh感染或CT感染),双因素感染、三因素感染。以活检结果为金标准,回顾分析和评估宫颈细胞DNA倍体阳性者中高危型HPV、UU/Mh和CT感染与宫颈上皮内瘤变之间的关系及风险性。结果DNA倍体阳性患者中慢性炎症占52.38%、CINI1级占22.02%、CIN1I级占10.71%、CINII级占14.88%;CINI1级中双因素感染最高占32.43%,CINⅡ级中高危型HPV感染最高占50.00%,CINBI级中双因素感染最高占56.00%、其次为高危型HPV感染占40.00%。结论宫颈细胞DNA倍体阳性合并HPV和UU/Mh双因素感染者及高危型HPV感染者应高度警惕为癌变高危人群。

Abstract:Objective To explore the relationship between cervical dysplasia and high risk type of human papillomavirus (HR-HPV). Methods 265 women of child-bearing age were involved in cervical cancer screening with DNA ploidy test. The women with positive result ( DNA aneuploidy) also tested by HPV test, UU/Mh test and CT test. We grouped them into infectious factors( HR-HPV +/UU/Mh +/CT + ) and without infectious factors. There are 3 situations in the infectious factors group: dual-factor infection (HR-HPV + combined with UU/Mh + ), tri-factor infection, HR-HPV infection or simple UU/Mh infection. The biopsy resuh is regarded to be the golden standard, reviewing the relationship between infectious factors, DNA aneuploidy cervical dysplasia to evaluate the risk of cervical intraepthelial'neoplasia (CIN). Results Among the biopsy result with DNA aneuploidy, the chronic cervicitis occupied 52. 38% , CIN I 22.02% , CIN II 10.71% and CIN III 14.88%. In the group with infectious factors, the ones with dual-factor infection in CIN I is 32.43% while 56.00% in CIN III as the most. Nevertheless, the most in CIN lI is HR-HPV as 50.00% , which emerged as 40.00% in CIN III. Conclusion The group with DNA aneuploidy combined with HR-HPV infection or HR-HPV and UU/Mh infection has the high potential risk of malignancy.
