侧脑室脑膜瘤的手术治疗

目的探讨侧脑室脑膜瘤的手术入路选择原则及神经导航系统、CUSA（cavitron ultrasonic surgical aspirator，超声外科吸引器）在侧脑室脑膜瘤切除中的作用。方法回顾性分析我院自1999年9月至2007年2月收治的23例侧脑室内脑膜瘤患者临床资料。结果手术全切23例，无手术死亡病例。术前症状改善21例、2例症状改善不明显。9例患者随访5个月至7年，无复发病例。随访病例中，1例术后出现视物变形者3个月后恢复正常，1例术前偏瘫（肌力Ⅰ级）患者2年后肌力明显增加（肌力Ⅲ级），7例无明显不适，正常生活工作。结论侧脑室内脑膜瘤首选显微手术治疗。术前周密计划，个体化选择手术入路加上神经导航系统及CUSA等手术辅助技术设备、工具的应用可以提高肿瘤的全切程度，减少手术并发症的发生。

Surgical Treatment of lateral ventricle meningiomas

Objective To explore the selection of the surgical approachs for lateral ventricle meningiomas and discuss the advantages of surgery supporting facilities,such as Neuronavigation system and CUSA(Cavitron Ultrasonic Surgical Aspirator).Methods 23 patients with lateral ventricle meningioma treated from March 1999 to February 2007 were retrospective investigated.All of them were undergone microsurgery,and Neuronavigation were used in 10 cases and CUSA in 11 cases.Results Total removal was achieved in all cases(100%)and no one died from operation.Postoperative headache occurred in 15 cases(65.2%).Aggravated or new neurological dysfunctions(17.4%) were found in 4 cases and all of them were cured.9 cases got followed up,and no recurrance was found.There was obvious improvement in 2 cases.7 cases live a normal life with no symptoms.Conclusions Microsurgery is the first choice for the treatment of lateral ventricle meningiomas.An individually chosen of proper approch and the using of surgery supporting facilities,such as Neuronavigation and CUSA,are much helpful to get total removal of the tumor and reduce the occurrence of postoperative complications.
