ECT全身骨显像对早期诊断骨转移肿瘤的临床价值

摘    要:目的 探讨发射型计算机断层显像(ECT)全身骨显像技术对恶性肿瘤骨转移的早期诊断价值.方法 对2012年10月至2013年9月间湖北医药学院附属东风医院收治的56例均经手术病理确诊为恶性肿瘤患者进行ECT全身骨显像检查,对检查结果进行分析.结果 56例恶性肿瘤患者ECT全身骨显像诊断结果显示,共46例检出肿瘤骨转移,检出率为82.14%,其中以乳腺癌的肿瘤骨转移检出率最高(90.91%,20/22),其次为肺癌和肝癌,检出率分别为87.50%(14/16)和80.00%(8/10),对前列腺癌的检出率最低(50.00%).ECT共检出肿瘤骨转移部位72处,其中以肋骨转移比例最高,为41.67%,其次是椎骨和四肢的检出率分别为25.00%和11.11%,其他部位的检出率合计为22.22%.ECT全身骨显像检查具有较高的敏感性(97.67%)和阴性预测价值(91.67%),但特异性较低(73.33%).结论 ECT全身骨显像在早期诊断肿瘤骨转移较传统影像学方法效果更好,可提前骨转移的检出时间,为患者赢得更宝贵的生存时间.

Clinical application of ECT whole body bone scan imaging technique in early diagnosis of bone metastasis
Abstract:Objective To assess the early diagnostic value of Emission Computed Tomography (ECT) whole body bone scan imaging technique in bone metastasis of malignant tumor. Methods During the period of October 2012 through September 2013, a total of 56 patients with malignant tumor were treated by whole body bone scan ECT in the Affiliated Dongfeng Hospital of Hubei University of Medicine, and their bone metastasis diagnostic condition was evalu-ated and analyzed. Results Of the 56 patients, 46 (82.14%) were diagnosed with bone metastases. The bone metastasis of breast cancer has the highest diagnostic rate for 90.91% (20/22), followed by lung cancer and liver cancer with 87.50%(14/16) and 80.00%(8/10), respectively, and prostate cancer with the lowest diagnostic rate for only 50.00%. A total of 72 bone metastasis places in 46 patients were found by ECT, among which rib metastases has the highest propor-tion for 41.67%, followed by the vertebrae and the limbs metastases (25.00%and 11.11%respectively), and other parts of metastases such as skull and pelvis (22.22%). The sensitiveness and negative predictive value of ECT in diagnosis of bone metastasis were 97.67%and 91.67%separately, and the specificity was 73.33%. Conclusion Compared with tradi-tional imageology methods, ECT whole body bone scan imaging technique has better performance in early diagnosis of bone metastasis of malignant tumor, which advance diagnostic time and win more precious survival time for patients.
