山楂治疗氯氮平所致肥胖高脂血症疗效观察

摘    要:目的:观察山楂治疗氯氮平所致肥胖高脂血症的临床疗效及安全性。方法采用随机数字表法将2013年1月至2014年12月我院精神科收治的86例精神分裂症患者分为观察组与对照组各43例,观察组应用氯氮平以及山楂丸治疗,疗程3个月,对照组仅应用氯氮平治疗,两组均每日辅助0.5 h心理行为干预治疗。分别于治疗的第0、1、2、3个月测查两组总胆固醇(TC)和甘油三酯(TG)、体质量和不良反应。结果观察组痊愈5例,显著进步15例,进步19例,无效4例,显效率为46.5%,有效率为90.7%;对照组痊愈2例,显著进步8例,进步12例,无效21例,显效率为23.3%,有效率为51.2%,观察组显效率、有效率均明显高于对照组,差异均有统计学意义(P<0.05);观察组的血脂TC、TG治疗前vs治疗后第2个月:(9.37±0.13) mmol/L vs (5.21±0.11) mmol/L、(2.92±0.12) mmol/L vs (2.21±0.13) mmol/L]、体重指数治疗前vs治疗后第3个月:(32.28±1.53) vs (24.21±1.73)]治疗后均降低,与治疗前比较差异均有统计学意义(P<0.05),对照组的血脂、体重指数治疗前后比较,差异均无统计学意义(P>0.05);两组治疗过程中不良反应TESS分值差异无统计学意义(P>0.05)。结论山楂治疗氯氮平所致肥胖高脂血症安全有效,服用方便依从性好。

Clinical effect of hawthorn in the treatment of obesity and hyperlipidemia induced by clozapine
Abstract:Objective To observe the clinical effect and safety of hawthorn in the treatment of obesity and hy-perlipidemia induced by clozapine. Methods Eighty-six patients of schizophrenia admitted in our hospital from Janu-ary 2013 and December 2014 were divided into two groups according to a random number table. The observation group (43 cases) was treated with clozapine and hawthorn pill for 3 months, and the control group (43 cases) was only treated with clozapine. The two groups were all applied daily psychological behavior intervention treatment for half an hour. The total cholesterol (TC) and triglyceride (TG), adverse reactions, and weight were detected before treatment, 1 month and 2, 3 months after treatment. Results After treatment, there were 5 cases of cured, 15 cases of significantly im-proved, 19 cases of improved, 4 cases of ineffective in the observation group, and 2 cases of cured, 8 cases of significant-ly improved, 12 cases of improved, 21 cases of ineffective in the control group. The markedly effective rate and the effec-tive rate in the observation group (46.5%, 90.7%) were significantly higher than those in the control group (23.3%, 51.2%), P<0.05. In the observation group, TC (9.37 ± 0.13) mmol/L before treatment vs (5.21 ± 0.11) mmol/L after 2 months of treatment], TG (2.92±0.12) mmol/L before treatment vs (2.21±0.13) mmol/L after 2 months of treatment], and weight index (32.28±1.53) before treatment vs (24.21±1.73) after 3 months of treatment] after treatment were all signifi-cantly decreased compared with those before treatment (P<0.05). TC, TG, and weight index showed no statistically sig-nificant difference in the control group between before and after treatment (P>0.05). TESS score of adverse reaction showed no statistically significant difference between the two groups (P>0.05). Conclusion Hawthorn is safe and effec-tive in the treatment of obesity and hyperlipidemia induced by clozapine, with good compliance.
