大黄祛伤膏与扶他林软膏治疗急性踝关节扭伤的临床对照研究

目的：比较大黄祛伤膏与扶他林软膏外用治疗急性踝关节扭伤的临床效果。方法将2014年7月至2015年7月我院中医骨伤科收治的60例急性踝关节扭伤患者以随机数字表法随机分为观察组和对照组，每组30例。观察组采用自制的大黄祛伤膏治疗，每3 d更换一次，两周为一个疗程。对照组采用扶他林软膏(20 g/支，北京诺华制药)涂抹患处，3次/d，两周为一个疗程。观察两组患者各种临床症状的改善情况，以及不良反应的发生情况。结果治疗两周后，两组患者各种症状均较治疗前有明显改善，差异均有显著统计学意义(P<0.05)；观察组在治疗瘀斑和肿胀方面优于对照组，差异均有统计学意义(P<0.05)；观察组患者临床控制6例，显效21例，有效3例，对照组临床控制3例，显效20例，有效7例，两组的总有效率相同，但观察组综合疗效优于对照组，差异均有统计学意义(P<0.05)；两组均未出现明显的不良反应。结论大黄祛伤膏与扶他林软膏治疗踝关节扭伤均有明确的疗效，但前者在治疗瘀斑和肿胀及综合疗效方面更有优势。

Comparison on the clinical effect of Dahuang Qushang Ointment with Votalin in the treatment of acute ankle ;sprain

Objective To observe the clinical curative effects of Dahuang Qushang Ointment, and Votalin (di-clofenac Diethylamine Emulgel) in the treatment of acute ankle sprain. Methods From July 2014 to July 2015, 60 pa-tients with acute ankle sprain in our hospital were divided into observation group (30 cases) and control group (30 cas-es). The patients in the observation group were treated with Dahuag Qushang Ointment (self-made) once three days for 5 times in two weeks, and the patients in the control group were treated with Votalin (20 g/piece, Novartis, Beijing) three times a day for two weeks. Results After two weeks of treatment, there were significant improvements in symptoms in the two groups compared with before treatment (P<0.01), and the observation group had better effect in dispersing swell than the control group (P<0.05). Six, 21, 3 patients in the observation group and 3, 20, 7 patients in the control group reached clinical control, markedly effective, effective, respectively. The total effective rates in the two groups were the same, but the comprehensive curative effects were better in the observation group than the control group (P<0.05). Conclusion The Dahuang Qeshang Ointment and Votalin both have certain effect in the treatment of acute ankle sprain, and the former is better than the latter in dispersing swell and comprehensive curative effects.
