妊娠期糖尿病孕妇血浆及脐血Betatrophin水平的变化及临床意义

摘    要:目的 探讨妊娠期糖尿病患者母血及脐血Betatrophin水平的变化,并分析其与临床、生化指标的相关性.方法 选取我院2013年6月至2015年6月收治的30例妊娠期糖尿病患者为研究对象(GDM组),30例正常健康妊娠妇女为对照(NGT组),采用酶联免疫吸附试验(ELISA)测定母血及脐血Betatrophin水平.结果 妊娠期糖尿病患者母血及脐血Betatrophin水平均明显高于正常妊娠妇女,差异有统计学意义(P<0.05);GDM组患者血清Betatrophin浓度与孕期体质量指数的增加量(ΔBMI)、胰岛素抵抗指数(HOMA-IR)、甘油三酯(TG)、总胆固醇(TC)及脐血Betatrophin呈正相关(r=0.344,P=0.026;r=0.317,P=0.041;r=0.364,P=0.018;r=0.644,P<0.001;r=0.352,P=0.022).结论 妊娠期糖尿病患者母血及脐血Betatrophin水平升高可能是胰岛素抵抗的一种代偿性机制,Betatrophin可能成为治疗各种类型糖尿病的新靶点.

Changes and clinical significance of Betatrophin level in plasma and cord blood in pregnant women with gestational diabetes mellitus
Abstract:Objective To investigate the changes of betatrophin level in plasma and cord blood of pregnant women with gestational diabetes mellitus (GDM), and to analyze the relationship between Betatrophin and clinical and biochemical indexes. Methods Thirty patients with GDM in our hospital between June 2013 and June 2015 were se-lected as the research subjects (GDM group). In addition, thirty healthy pregnant women in the same period were select-ed as the control group (NGT group). Enzyme linked immunosorbent assay (ELISA) was applied to determine the levels of Betatrophin in plasma and cord blood. Results The Betatrophin levels in plasma and cord blood in GDM group were significantly higher than those in NGT group (P<0.05). Betatrophin concentrations in the GDM group were posi-tively correlated with the increase in body mass index during pregnancy (ΔBMI), insulin resistance index (HOMA-IR), triglyceride (TG), total cholesterol (TC) and betatrophin levels in cord blood (r=0.344, P=0.026; r=0.317, P=0.041; r=0.364, P=0.018;r=0.644, P<0.001;r=0.352, P=0.022). Conclusion Elevated levels of Betatrophin in plasma and cord blood of GDM patients may be a compensatory mechanism of insulin resistance, and Betatrophin might serves as a new target for the treatment of various types of diabetes.
