塑形取石网篮在胆总管困难结石所致胆道梗阻治疗中的应用价值

目的 探讨经加工塑形的取石网篮在胆总管困难结石所致胆道梗阻治疗中的临床应用价值.方法 对194例胆总管困难结石所致胆道梗阻患者采用内镜逆行胰胆管造影术(ERCP)进行内镜下取石术,经插管造影明确诊断后,应用经加工塑形的取石网篮治疗102例,采用普通取石网篮治疗92例.结果 176例患者一次性取石成功,其中97例应用塑形后的取石网篮,其成功率为95.1%,80例应用普通取石网篮,成功率为86.9%,两者比较差异有统计学意义(P＜0.05);两组一次取石成功所需时间(65.8±40.2)min vs(85.7±65.4)min,两组比较差异具有统计学意义(P＜0.05).结论 应用经塑形取石网篮在困难胆总管结石所致胆道梗阻治疗中既省时,又提高成功率,安全有效.

Value of remodel basket in treatment of biliary obstruction with difficult choledocholithiasis by endoscopic retrograde cholangiopancreatography

Objective To explore the clinical value of the remodel basket in biliary obstruction with difficult choledocholithiasis by endoscopic retrograde cholangiopancreatography.Methods Difficult choledocholithiasis of 194 patients were removed by endoscopic retrograde cholangiopancreatography,the patients of choledocholithiasis were definitely diagnosed by ERCP,and they were randomly divided into treatment group and control group,102 patients in treatment group were treated by the remodel basket,other 92 patients in control group were treated by common basket.Results Difficult choledocholithiasis of 176 patients were removed completely one time,the successful rate was 95.1% in treatment group and 86.9% in control group（P〈0.05）.The entire time of removing stones was（65.8±40.2） min in treatment group and（85.7±65.4） min in control group（P〈0.05）.Conclusion Difficult choledocholithiasis to remove with the remodel basket is not only improving the successful rate but also saving time,the method is save and effective.
