慢性乙型肝炎患者外周血树突状细胞免疫功能的研究

目的 研究慢性乙型肝炎(CHB)患者外周血树突状细胞(DC)激活及促进T淋巴细胞分化的免疫功能.方法 采用体外细胞培养的方法培养30例CHB患者及20例健康体检者外周血DC,用流式细胞术检测CD83及CD86的表达,甲基噻唑基四唑(MTT)法检测DC对混合淋巴细胞的刺激作用,酶联免疫吸附测定方法栓测培养上清中白细胞介素12(IL-12)及白细胞介素6(IL-6)的含量.结果 CHB患者DC中CD83、CD86的表达量分别为0.118±0.036和0.179±0.035,健康体检者则分别为0.374±0.057和0.385±0.050(均P＜0.01).CHB患者和健康体检者的DC对混合淋巴细胞的刺激指数分别为0.069±0.042和0.101±0.040(P＜0.01).CHB患者组DC培养上清中IL-12、IL-6的含量分别为(38.132±11.581)ng/L和(1 843.366±235.515)ng/L,健康体检者分别为(311.400±24.707)ng/L和(117.025±61.491)ng/L,两者差异有统计学意义(均P＜0.01).结论 CHB患者外周血DC提呈抗原、激活T淋巴细胞的功能明显低下,且使T辅助细胞0(Th0)向T辅助细胞1(Th1)分化的能力减弱.

Study on immune functions of peripheral blood dendritic cells in patients with chronic hepatitis B

Objective To investigate the capability of peripheral blood dendritic cells（DC） in patients with chronic hepatitis B（CHB） to stimulate the proliferation and differentiation of T lymphocytes in vitro.Methods Peripheral blood DC in 30 patients with CHB and 20 healthy individuals was cultured.CD83 and CD86 on DC were analyzed by flow cytometry.The capability of DC to stimulate T lymphocyte proliferation was evaluated by allogenetic mixed lymphocyte reaction.The levels of IL-12 and IL-6 in cell culture supernatant were detected by enzyme-linked immunosorbent assay（ELISA）.Results The expression of CD83 and CD86 on DC in patients with CHB were（0.118±0.036） and（0.179±0.035）,but they were（0.374±0.057） and（0.385±0.050） in healthy individuals（all P0.01）,respectively.Stimulation index of DC to T lymphocytes was（0.069±0.042） and（0.101±0.40） in patients with CHB and healthy individuals（P0.01）,respectively.Concentrations of supernatant IL-12 and IL-6 were（38.132±11.581） ng/L and（1 843.366±235.515） ng/L in patients with CHB,but（311.400±347.07） ng/L and（117.025±61.491） ng/L in healthy individuals（all P0.01）,respectively.Conclusion The capabilities of antigen presentation and activating T lymphocytes of DC in patients with CHB are significantly decreased,and the capability of promoting differentiation from T helper cell 0（Th0） to T helper cell 1（Th1） is impaired.
