糖尿病视网膜病变的彩色多普勒超声研究

目的：探讨糖尿病视网膜病变（DR）患者眼球后动脉血流动力学的变化规律,及早发现糖尿病眼部并发症,并分析DR病程进展的相关因素.方法：应用彩色多普勒超声检测糖尿病患者165例（330眼）和正常对照者132例（264眼）的眼动脉（OA）和视网膜中央动脉（CRA）的收缩期峰值血流速度（PSV）、舒张末期血流速度（EDV）、阻力指数（RI）和脉动指数（PI）.结果：DR患者CRA的PSV、EDV、OA的EDV较正常对照组显著降低,RI和PI则显著增高,有显著差异（P＜0.01）.随着患者视网膜病变程度的加重,血流速度逐渐减低,RI、PI逐渐增高.在DR发生过程的极早期阶段,CRA的EDV下降,RI和PI明显升高,与正常对照组比较均有显著差异（P＜0.01）.结论：可利用CDFI诊断糖网病,血流参数的变化可以动态监测视网膜病变的程度及发展,在无视网膜病变的糖尿病患者视网膜血流动力学已发生变化,可极早期诊断DR.

Along with the patient retina pathological change degree aggravation, the blood stream speed decreases gradually, RI, PI advance gradually. Has the process side early time stage in DR,the CRA EDV drop,RI and the PI obvious ascension,quite is even with the normal control group has the remarkable difference （P〈0.01）. Conclusion ; May use the CDFI diagnosis sugar net sickness,the blood stream parameter change may the dynamic monitor retina pathological change degree and the development,is disregarding the net interface pathological change diabetic retina blood stream dynamics to change,may the early time diagnose DR extremely.
