保留棘突节段性扩大减压术治疗腰椎管狭窄症

本文介绍了自行设计保留棘突韧带结构,节段性扩大减压术治疗腰椎管狭窄症22例.该手术是经过一侧显露棘突及椎板,再由棘突基底部向对侧水平凿开手术范围内的棘突,并向对侧牵开棘突及骶棘肌,显露全椎板及小关节突,保留椎板中1/3椎管环完整,进行节段性扩大减压,最后将牵开的棘突韧带结构复位,并缝合固定.术后经6个月至4年随访19例,优良率达95%.作者认为该手术既能充分减压,又可保持脊柱的稳定性和活动范围.

Treatment of Lumbar Spinal Canal Stenosis by Enlarged Decompression in Some Segments and Conserved Spinous Processes

This article introduced the method designed by the author that conserved spinous process ,supraspinal, interspinal ligament and vertebral canal ring,enlarged decompression in some segments for 22 patients of lumbar spinal canal stenosis. The operation exposured spinous processes and vertebral lamina from unilateral,then chiseled spinous processes from its base to the opposite side in the operative segment,drow spinous process and sacrospinal muscle to the opposite side,exposured all vertebral lamina and processes of small articulations,conserved the inte-graty of vertebral lamina,enlarged decompression in a segment or several segments,then recovered the construction of ligament of spinous processes,sutured and fixed. Ninteen patients were followed six months to four years after the operation. The excellent rate was 95%. We think the operation can make full decompression and conserve stability and mobility of spinal column.
