摘    要:目的探讨同伴教育在男男性行为者(MSM)人群中的知识认知和行为转变的效果。方法2004年3~11月,在昆明、成都两市的MSM人群中开展有组织的同伴教育活动;在4月和11月,分别进行性病、艾滋病知识和个人性行为情况的人群抽样调查,对调查结果进行比较分析。结果基线调查184人,终期调查175人。两次调查发生首次性行为的年龄中值均为18岁,发生首次同性性行为年龄的中值均为19岁。基线和终期调查发现,调查对象对性病、艾滋病知识的认知发生了有意义的全面提高,但在近3个月内异性性行为和同性性行为的发生率和性伴数没有改变,并且平均性伴数都超过1人。各种方式性行为的发生率没有改变,安全套的使用率也没有发生有意义的提高,只是对性对象的健康状况关注增加了。结论同伴教育能够很快提高MSM群体的性病、艾滋病知识认知,但是存在“认知与行为分离”的现象,加速安全性行为的促进是面临的重要问题,同时安全套促进也需要人性化措施。

Abstract:Objective To investigate effects of the peer education on the change of STD/AIDS knowledge and related behaviors among men who have sex with men(MSM).Methods An organized peer education was carried out among MSM in the cities of Kunming and Chengdu from March to November,2004.The knowledge and behaviors were randomly sampled from the MSM population in April and November separately,and then the findings were analyzed.Results There were 184 MSM recruited at the baseline survey,and 175 at the terminal survey.The median age of first sex and first homosexual contact was 18 and 19 years in the two surveiys,respectively.It was found that the STD/AIDS knowledge was singnificantly improved in the terminal survey.However,there were no changes in the frequency of heterosexual and homosexual behaviors and the number of sex partners(more than one partner on average)in last three months.The pattern of sex and use of condoms have not significantly changed either,but more attention was paid to care for their sex partners.Conclusion Although the knowledge of STD/AIDS can be enhanced greatly by the peer education in a short time,there is a phenomenon that the knowledge does not square with the related behaviors.Therefore,it is necessary to provide safe sex education continuously and the condom promotion has to fit with human feelings.
