176例多导睡眠呼吸监测结果及临床分析

目的]探讨不同程度阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)和单纯鼾症在临床表现、睡眠监测指标上的差异。方法]将176例患者分为单纯鼾症组,OSAHS轻、中、重度组,记录其年龄、性别、体重指数(BMI)、临床症状、多导睡眠监测结果、合并症(高血压、糖尿病、心脏病等)出现率,并分析比较其临床症状及合并症出现的差异。结果]30～60岁患者占总患者数的82.95%。各组之间临床表现和合并症出现率的差异均有显著性意义(P<0.05)。BMI、最大氧减(%)、微觉醒指数(MAI)与呼吸暂停低通气指数(AHI)显著正相关(r分别为0.398、0.772、0.567,P均<0.01);平均血氧饱和度(MSaO2)与AHI显著负相关(r=-0.623,P<0.01)。结论]30～60岁为OSAHS高发年龄,但其严重程度与年龄分布无关。不同程度OSAHS和单纯鼾症在临床表现、睡眠监测指标上均有明显差异,可通过其判断患者病情及预后。

Polysomnography respiratory monitoring results and clinical analysis of176 Cases

Objective] Explore difference between obstructive sleep apnea syndrome(Obstructive sleep apnea hypopnea syndrome,OSAHS) and simple snore in clinical manifestation,sleep in monitoring index.Methods] 176 cases were divided into pure snore group,OSAHS light groups,medium groups,heavy groups,record its age,gender,body mass index(BMI),the clinical symptoms,polysomnography monitoring results,complications,such as hypertension,diabetes and heart disease) morbidity,analyzed and compared the clinical symptoms and the consolidated the difference there.Results] 82.95% of the total number of patients are age 30 to 60 year old.There are significant differences among groups(P<0.05) in the clinical manifestations and complications of rate between are BMI,maximal oxygen reduction(%),arousal index(MAI) and the apnea-hypopnea index(AHI) has a significant positive correlation(r=0.398,0.772,0.567,P<0.01);the mean oxygen saturation(MSaO2) with AHI has a significant negative correlation(r=-0.623,P<0.01).Conclusion] Age 30 to 60 years old is high-risk age for OSAHS,but its severities are uncorrelated with the age distribution.There are obvious difference between different degree OSAHS and simple snore clinical manifestation,sleep in monitoring index all have,which can be used for diagnosis and prognosis.
