关节镜下逆行交锁髓内钉治疗股骨髁上骨折

目的探讨关节镜监视下逆行交锁髓内钉治疗股骨髁上骨折的效果。方法从1999年6月至2000年12月，在关节镜监视下以逆行交锁髓内钉技术结合闭合复位治疗17例股骨髁上骨折。结果本组获得随访11例，随访时间半年以上，平均愈合时间为3个月。所有患者膝关节屈伸活动均>90°，无一例发生感染，无一例内固定物断裂。结论本法结合了关节镜和逆行交锁髓内钉的优点，手术时间短、出血少、创伤小，最大程度地保留了关节部位解剖与功能的完整，同时提供了坚强固定，有利于骨折的愈合和膝关节功能的恢复。

Effects of retrograde interlocking intramedullary nails under arthroscopy on supracondylar femoral fractures

Objective　To evaluate the therapeutic effects of retrograde interlocking intramedullary nails under arthroscopy on supracondylar femoral fractures.　Methods　From June 1999 to December 2000, 17 patients with supracondylar femoral fractures were treated with arthroscopically assisted retrograde placement of interlocking intramedullary nails and close reduction. 　Results　Followed up for more than 6 months in 11 patients, it revealed that the average healing time was 3 months. The average range of the knee motion for all the patients was more than 90°. There was no implant breakage and infection. 　 Conclusions　 This new method, combining the advantage of arthroscope and retrograde interlocking intramedullary nail, can provide a stable and reliable fixation, and meanwhile, is less invasive to the soft tissues and knees, less operative time, less blood loss and minimal disruption of the blood supply in the fracture site. It is conducive to the fracture healing and the functional recovery of the knee joint and worthwhile to be recommended.
