瞬时加速度波强与左心室收缩功能关系的研究

摘    要:目的通过对瞬时加速度波强(accelerating wave intensity,W_1)与LVEF、左心室压力最大上升速度(+dp/dt_(max))及6分钟步行距离(6 minutes walking distance,6 MWD)的比较,探索W_1与左心室收缩功能之间的关系。方法选择冠状动脉造影证实至少1支血管一处狭窄50%的患者81例,应用日本ALOKA公司ProSound α10彩色多普勒超声诊断仪检测患者W_1、LVEF,同时检测6 MWD,并于2 h内通过心导管检查测左心室+dp/dt_(max)。将入选者按LVEF水平分为3组:Ⅰ组56例,LVEF≥50%;Ⅱ组15例,35%≤LVEF50%;Ⅲ组10例,LVEF35%。结果与Ⅰ组比较,Ⅱ组和Ⅲ组W_1明显下降;与Ⅱ组比较,Ⅲ组W_1明显下降,差异有统计学意义(P0.05)。W_1值与LVEF、6 MWD及左心室+dp/dt_(max)均呈正相关(r=0.485、r=0.555、r=0.691,P0.01)。结论 W_1可作为反映左心室收缩功能的一项超声指标。

The relationship between W1 and left ventricular systolic function
Abstract:Objective To explore the relationship between accelerating wave intensity(W_1) and left ventricular systolic function in patients with coronary artery disease (CAD) by analysis of W_1, LVEF,LV + dp/dt_(max) and six minutes walking distance(6 MWD).Methods Ninety patients with CAD were examined with Prosound a 10 color Doppler ultrasonograph(ALOKA,Tokyo,Japan) , the W_1 and LVEF were recorded.After echocardiography,the 6 MWD of every patient was measured. Coronary angiography was taken within 2 hours,more than 50% stenosis in one vessel was found at least in 81 patients.Then left ventricular + dp/dt_(max) was recorded in 47 patients.The 81 patients were divided into three groups according to the level of LVEF.Results In patients with LVEF<50%,Wi decreased significantly compared with the group with normal LVEF.W_1 was positively correlated with LVEF,6 MWD and LV + dp/dt_(max)(r = 0.485,r= 0.555,r = 0.691,P< 0.01).Conclusions W_1 can be used to evaluate the left ventricular systolic function in patients with CAD.
