摘    要:目的 评价心肌肌钙蛋白T(cTnT)对心绞痛(AP)患者预后的判断价值。方法 采用酶联免疫吸附法(ELISA)动态测定77例心绞痛患者cTnT浓度,同时进行心肌磷酸肌酸激酶(CK)和磷酸肌酸激酶同功酶(CK—MB)测定。结果50例稳定性心绞痛(SAP)患者无1例血清cTnT异常升高,而27例不稳定性心绞痛(UAP)患者,11例血清cTnT异常升高,其中6人发展为急性心肌梗死。结论cTnT预测心绞痛患者预后的阳性预期值为54.5%,阴性预期值为100%。而CK—MB则相应为28.6%和80%,均低于血清cTnT。故在筛选UAP高危病人时,可通过血清cTnT增高加以区分。

Abstract:Objective To evaluate the value of evaluation of prognosis for patients with angina pectoris by detecting the troponin T in serum. Methods The study was conducted continuously and 77 patients with angina pectoris was included. Their serum cTnT levels were consecutively detected using enzyme-linked immunosorbent assay (ELISA). Whilst CK and CK-MB levels were measured and modestly assessed. Results None of the 16 stable AP ( SAP ) patients experienced a considerably and significantly increasing concentration of serum cTnT but 11 of the 27 unstable AP (UAP) patients had deviant cTnT levels in serum and then 6 of them developed AMI within 2 to 10 days after the positive in this clinical trial. Conclusion The positive anticipation index which was prognosticated using cTnT is 54. 5% and the negative one is 100% but this index prognosticated using CK-MB is 28. 6% and 80% respectively . So the serum cTnT is an usable indicator to the resolution of patients running high risks.
