肝门部胆管癌手术方式的选择

目的 探讨肝门部胆管癌的外科手术方式。方法 对近6年来收治的31例肝门部胆管癌病例的手术方式进行回顾性分析。结果 31例手术治疗的病例中，行根治性切除7例。姑息性切除5例，总切除率为38．7％。行各种引流术17例，包括外引流2例，内引流15例，占总数的54．8％。探查术2例，占总数的6．5％。结论 肝门部胆管癌一旦确诊，应积极手术治疗，尽管有根治性切除率低、手术风险大等，但即使姑息性切除，甚至引流术也可有效解除黄疸、提高病人生活质量和延长生存期。

OPTION OF SURGICAL TREATMENT FOR HILAR CHOLANGIOCARCINOMA

Objective To evaluate the surgical methods for hilar cholangicarcinoma.Methods The surgical methods were retrospectively analysed on 31 cases of hilar cholangiocarcinoma admitted into our hospital during the last six years.Result 7 cses were underwent radical resection.5 cases were underwent palliative resection.Total resecting rate was 38.7%.Various drainage were performed on 17 cases including external drainage in 2 cases and internal drainage in 15 cases was 54.8% of total.2 cases were underwent exploration,which was 6.5% of total.Conclusion Once hilar cholangiocarcinoma was diagnosed,surgical treatment should be performed,though lower radical resection rate and high surgical risk existed.Palliative resection,and drainage procedures can relieve jaundice effectively,improve patient's life quality and prolong survival period.
