摘    要:目的 初步探讨1.5T磁共振PROSET与VISTA序列对正常人腰骶脊神经显示的临床应用价值.方法 选择2016年8~11月期间来杨凌示范区医院的无症状正常志愿者32例行MRI扫描,包括MRI常规序列、PROSET及VISTA序列,将PROSET及VISTA序列所得原始图像分别进行后处理,并对马尾神经、脊神经根及固有脊神经的显示情况进行评分.采用两个相关样本比较的Wilcoxon检验,比较两种扫描序列对各段脊神经显示评分的差异.结果 马尾神经显示的VISTA序列评分为(4.25±0.762)分,明显高于PROSET序列的(1.84±0.677)分,差异有统计学意义(P<0.01);脊神经根显示的PROSET序列评分为(4.06±0.716)分高于VISTA序列的(3.66±0.787)分,差异有统计学意义(P<0.05);固有脊神经显示PROSET序列评分为(3.16±0.677)分,与VISTA序列的(3.03±0.595)分比较差异无统计学意义(P>0.05).结论 PROSET与VISTA序列对腰骶脊神经各段的显示各有所长,联合使用能清晰显示各段脊神经形态,结合MRI常规序列,为临床腰骶脊神经病变的诊治提供准确的影像学依据.

Abstract:Objective To investigate the clinical value of MR principle of selective excitation technique (PRO-SET) and volume isotropic turbo spin echo acquisition (VISTA) sequences in the lumbosacral spinal nerves display on 1.5T MRI. Methods From August 2016 to November 2016, MRI scans, including routine MRI, PROSET and VISTA sequences, were performed on 32 asymptomatic normal volunteers from Yangling Demonstration Zone Hospital. Image post-processing were performed on the original data obtained from PROSET and VISTA sequences. The original images of PROSET and VISTA sequences were post-processed, and the results of cauda equina, spinal nerve root and intrinsic spinal nerve were evaluated. Wilcoxon test was used to compare the differences of the scores of the spinal nerves dis-played by the two scanning sequences. Results The score of cauda equina display was (4.25 ± 0.762) for VISTA se-quence versus (1.84 ± 0.677) for PROSET sequence (P<0.01). The score of spinal nerve roots display was (4.06 ± 0.716) for PROSET sequence versus (3.66 ± 0.787) for VISTA sequence (P<0.05). There were significant differences between the two sequences (P<0.05). There was no significant statistical difference between PROSET (3.16 ± 0.677) and VISTA (3.03 ± 0.595) sequences in intrinsic spinal nerves display (P>0.05). Conclusion PROSET and VISTA sequences each has its own advantages in lumbosacral spinal nerves display. The joined application of PROSET and VISTA sequences can directly visualize the paths of lumbosacral spinal nerves, which combined with routine MR images can provide imag-ing evidence for clinical diagnosis and treatment.
