儿童幽门螺杆菌感染与缺铁性贫血的相关性

摘    要:目的 研究儿童幽门螺杆菌感染与缺铁性贫血的相关性.方法 选取我院2015年1月至2016年8月收治的120例儿童,依据胃镜及Hp检测结果将这些患儿分为Hp阳性慢性胃炎组(n=40)、Hp阳性胃镜下无损害组(n=40)和Hp阴性慢性胃炎组(n=40)三组,对三组患儿的缺铁性贫血指标、缺铁性贫血发生情况进行统计分析.结果 Hp阳性慢性胃炎组、Hp阳性胃镜下无损害组患儿的血红蛋白(Hb)、平均红细胞体积(MCV)、血清铁(SI)、血清铁蛋白(SF)水平[(113.3±11.4)g/L、(77.5±6.5)fl、(12.3±11.5)μmol/L、(16.5±8.2)μg/L和(116.3±13.2)g/L、(76.6±6.5)fl、(12.5±6.5)μmol/L、(20.3±12.5)μg/L]均显著低于Hp阴性慢性胃炎组[(133.5±7.5)g/L、(87.2±6.3)fl、(19.5±5.3)μmol/L、(35.2±11.5)μg/L](P<0.05),总铁结合力(TIBC)水平[(67.5±18.2)mmol/L、(73.5±18.2)mmol/L]均显著高于Hp阴性慢性胃炎组[(54.2±15.2)mmol/L],缺铁性贫血发生率[42.5%(17/40)、47.5%(19/40)]均显著高于Hp阴性慢性胃炎组[10.0%(4/40)],差异均有统计学意义(P<0.05),但Hp阳性慢性胃炎组、Hp阳性胃镜下无损害组患儿的Hb、MCV、SI、TIBC、SF水平、缺铁性贫血发生率比较差异均无统计学意义(P>0.05).结论 儿童幽门螺杆菌感染与缺铁性贫血密切相关,值得临床充分重视.

Abstract:Objective To study the correlation between Helicobacter pylori infection and iron deficiency anemia in children. Methods A total of 120 cases of children, who admitted to our hospital from January 2015 to August 2016, were selected and divided into the Hp positive chronic gastritis group (n=40), the Hp positive under gastroscope without damage group (n=40) and Hp negative chronic gastritis group (n=40) according to gastroscopy and Hp detection results. The iron deficiency anemia indexes, incidences of iron deficiency anemia of the three groups were statistically analyzed. Results The hemoglobin (Hb), mean corpuscular volume (MCV), serum iron (SI), serum ferritin (SF) levels of the Hp positive chronic gastritis group and Hp positive under gastroscope without damage group were respectively (113.3 ± 11.4) g/L, (77.5±6.5) fl, (12.3±11.5)μmol/L, (16.5±8.2)μg/L and (116.3±13.2) g/L, (76.6±6.5) fl, (12.5±6.5)μmol/L, (20.3±12.5)μg/L, which were significantly lower than (133.5±7.5) g/L, (87.2±6.3) fl, (19.5±5.3)μmol/L, (35.2±11.5)μg/L in the Hp negative chronic gastritis group (P<0.05); the total iron binding capacity (TIBC) levels were respectively (67.5±18.2) mmol/L and (73.5±18.2) mmol/L, which were significantly higher than (54.2±15.2) mmol/L of the Hp nega-tive chronic gastritis group (P<0.05); the incidence of iron deficiency anemia respectively 42.5% (17/40) and 47.5%(19/40), which were significantly higher than 10.0%(4/40) of the Hp negative chronic gastritis group (P<0.05). There was no significant difference between the Hp positive chronic gastritis group and Hp positive under gastroscope with-out damage group in the levels of Hb, MCV, SI, TIBC, SF and the incidences of iron deficiency anemia (P>0.05). Conclusion Helicobacter pylori infection is significantly associated with iron deficiency anemia in children, which is worthy of the clinical attention.
