经阴道彩色多普勒超声诊断剖宫产瘢痕处早期妊娠的临床价值

目的 探讨经阴道彩色多普勒超声诊断子宫瘢痕处早期妊娠的价值.方法 选择商洛市中医院医院2010年1月至2016年1月期间收治的86例子宫瘢痕处早期妊娠患者作为研究对象,采用数字表法随机分为观察组和对照组,每组43例,观察组予经阴道超声检查,对照予经腹部超声检查,比较两组患者的诊断准确率及漏诊率.结果 根据超声声像图特征,将剖宫产瘢痕处早期妊娠分为孕囊型、混合回声包块型和孕囊部分位于宫腔型.观察组诊断准确率为90.7%(40/43),明显高于对照组的67.4%(29/43),差异有统计学意义(P<0.05);观察组漏诊率为2.3%(1/43),明显低于对照组的18.6%(8/43),差异有统计学意义(P<0.05);两组患者各型剖宫产瘢痕处早期妊娠检出率比较差异均无统计学意义(P>0.05).结论 经阴道彩色多普勒超声检查对剖宫产后瘢痕处妊娠的临床诊断准确率高,漏诊率低.

Clinical value of transvaginal color Doppler ultrasound in the diagnosis of early pregnancy in cesarean scar

Objective To discuss the application value of transvaginal color Doppler ultrasound in the diagnosis of pregnancy in cesarean section scar. Methods A total of 86 cases of early pregnancy patients with uterine scar, who ad-mitted to Shangluo Hospital of Traditional Chinese Medicine from January 2010 to January 2016, were selected and divid-ed into the observation group and the control group according to random number tables, with 43 cases in each group. Ab-dominal ultrasound was used in the control group, and transvaginal color Doppler ultrasound was used in the observation group. The diagnostic accuracy and misdiagnosis of the two groups were compared. Results According to the character-istics of ultrasonography, the early pregnancy in cesarean section scar was divided into gestational sac type, mixed echo type and gestational sac in the uterine cavity. The diagnostic accuracy was 90.7%(40/43) in the observation group, which was significantly higher than 67.4%(29/43) in the control group (P<0.05);the misdiagnosis rate was 2.3% (1/43) in the observation group, which was significantly lower than 18.6% (8/43) in the control group (P<0.05). There was no significant difference between the two groups in the detection rate of early pregnancy in ce-sarean section scar (P>0.05). Conclusion Transvaginal color Doppler ultrasonography has a high accuracy in diag-nosis of scar pregnancy after cesarean section, and a low rate of missed diagnosis, which is worthy of clinical application.
