摘    要:目的 探讨门诊机会筛查妇女的人乳头瘤病毒(HPV)感染的流行病学特点,为西安地区宫颈病变的防治提供依据.方法 收集2013年1月至2015年1月在西安交通大学第一附属医院妇科门诊接受HPV检测的8578例女性患者资料,分析HPV感染率及亚型分布特点.结果 医院机会筛查妇女HPV总阳性率为30.45%(2612/8578),其中高危型2319例,占88.78%,排在前5位的亚型依次为16、52、58、31、18型;低危型293例,占11.22%,排在前3位的亚型依次为6、CP8304、11型.HPV单一感染的阳性率为21.90%,多种感染率为8.55%,以二重感染为主.HPV感染的年龄分布呈"U"型特点,≤25岁及≥55岁年龄组HPV感染率与其余组的差异具有统计意义(P<0.05).结论 西安地区HPV感染及其亚型分布具有明显的地域特点,以高危型感染为主;尤其对于≤25岁和≥55岁女性,应加强推行女性健康教育和全面的宫颈疾病筛查措施.

Abstract:Objective To investigate the epidemiological characteristics of HPV infection among opportunistic screening in outpatient, and to provide the theory basis for prevention and intervention of cervical precancerous lesions in Xi'an. Methods The data of 8578 female patients, who underwent HPV typing test at the Outpatient Department of Gynecology&Obstetrics in our hospital from January 2013 to January 2015, were collected. The infection rate and sub-type distribution characteristics of HPV were analyzed. Results The total HPV positive infection rate of the screen-ing crowd was 30.45% (2612/8578), among which, 2319 (88.78%) positive cases belonged to the high-risk HPV (HR-HPV) infection, and the top five subtypes were HPV16, 52, 58, 31, 18. Atotal of 293 (11.22%) positive cases be-longed to low-risk type HPV, and the top three subtypes were in order for HPV6, CP8304, HPV11. The positive rate of single HPV infection was 21.90%, and the positive rate of multiple infections was 8.55%, in which double infection was the main infection model. The characteristics of age distribution of HPV infection like a"U". The differences of HPV in-fections rate between women less than 25 years old or over the age of 55 and the other groups were statistically signifi-cant (P<0.05). Conclusion The distribution of HPV infection and subtypes of Xi'an was region specific, and the most of females were infected by high-risk subtype HPV. Comprehensive health education and cervical cancer screening pro-gram should be strengthen promoted, especially for the women who under 25 years old or more than 55 years old.
