序贯法治疗水平半规管管结石症及壶腹嵴帽结石症

目的：探讨Barbecue法、Gufoni法联合强迫长时间侧卧位法（FPP）序贯治疗在水平半规管管结石症和壶腹嵴帽结石症中的临床应用价值。方法：分析2010—02—2012—08期间确诊为水平半规管良性阵发性位置性弦晕的42例患者，确诊为管结石症者首先行Barbecue法复位，嵴帽结石症患者则先行Gufoni法转换眼震后行Barbecue法复位。对无效和不能耐受Barbecue复位者联合FPP。结果：42例患者中，首次手法复位成功率为76．2％（32／42）。首次治疗后无效、复发和不能耐受者共16例，经Barbecue复位法和（或）Gufoni法联合FPP序贯治疗后有效率为87．5％（14／16）。结论：Barbecue法、Gufoni法联合FPP序贯法是治疗不能耐受反复Barbecue复位、无效以及复发性水平半规管结石症患者的一种实用、有效的方法。

Sequential therapy to treat the canalolithiasis and cupul,olithiasis of horizontal semicircular canal

Objective：To evaluate sequential therapy in the clinical applications of the Barbecue,Gufoni maneu- ver combined with forced prolonged position（FPP） in the treatment of the canalolithiasis and cupulolithiasis of hor- izontal semicircular canal. Method.. Forty two patients of HSC-BPPV collected from February 2010 to August 2012 were analyzed retrospectively, the canalolithiasis＇s patients were treated with the Barbecue maneuver firstly, the cupulolithiasis＇s patients turned into geotropic nystagmus with Gufoni maneuver before the Barbecue maneuver, the unresolved and intolerated of Barbecue maneuver cases received FPP. Result：The initial success rate of all patients was 76.2% （32/42）. There were 16 cases unresolved, recurrent and intolerated after initial treatment. By sequen- tial therapy, the effective rate was 87.5% （14/16）. Conehlsion：Sequential therapy of the barbecue,Gu/oni maneu- ver combined with FPP, is a practical and effective method for the patients of horizontal semicircular canal who un- resolved and recurrent and intolerated of the Barbecue maneuver.
