股骨转子间骨折动力髋螺钉及近端锁定钢板治疗效果分析

目的探讨动力髋螺钉(DHS)及股骨近端锁定钢板治疗股骨转子间骨折的临床效果,为其治疗寻找最佳方法。方法将80例股骨转子间骨折患者采用随机数字的方法分为观察组和对照组各40例,观察组采用DHS治疗,对照组采用股骨近端锁定钢板治疗,观察两组患者手术时间、术中出血量、术后住院时间、骨折愈合时间和并发症,以及术后1年采用Harris髋关节功能评价标准评定患者术后髋关节功能。结果观察组患者手术时间和术中出血量均优于对照组(P<0.05),两组患者术后住院时间、骨折愈合时间和并发症发生率无显著性差异(P>0.05);观察组优良率为90.00%,明显高于对照组的75.00%(P<0.05)。结论 DHS治疗股骨转子间骨折手术创伤小、固定牢靠、抗旋转性强,术后髋关节功能恢复好,是治疗股骨转子间骨折的理想方法之一。

Clinical Observation of Femoral Intertro-chanteric Fractures with Dynamic Hip Screw and Proximal Locking Plate

Objective To discuss treatment efficacy of combination of dynamic hip screw（DHS） and proximal locking plate in patients with femoral intertro-chanteric. Methods 80 patients with femoral intertro-chanteric were divided randomly into observation group （40 eases） and control group （40 cases） . The observation group was treated with DHS, and the control group was treated with proximal locking plate. The operative time, blood loss, hospital stay and fracture healing time, complications were recorded. To assess hip function with the Harris hip function evaluation criteria after 1 year. Results The operative time and blood loss in the observation group was better than the control group （P〈0.05）, there is no significant difference for the hospital stay and fracture healing time, complications（P〉0.05）. The effective rate of the observation group （90.00%） was higher than that of the control group （75.00%） （P〈0.05）. Conclusion DHS is the treatment of femoral intertro-chanteric fractures of the ideal methods, for it possesses the advantages of less surgical trauma, strong fixation, anti-rotation, and good functional recovery after hip.
